摘要
目的:对比生物型髋关节置换术(BHA)、股骨近端防旋髓内钉(PFNA)内固定术治疗高龄不稳定股骨粗隆间骨折患者的效果。方法:将2022年1月—2024年12月德江县人民医院收治的70例高龄不稳定股骨粗隆间骨折患者按照手术方法分为对照组(32例,PFNA内固定术)、观察组(38例,BHA)。对比两组手术指标、术后并发症发生率、髋关节功能、日常生活能力。结果:观察组手术时间长于对照组(P<0.001),术中出血量、术后引流量高于对照组(P<0.001),术后首次下床活动时间、负重时间短于对照组(P<0.001);观察组术后并发症发生率低于对照组(P=0.011);术后1、3、6个月,观察组Harris评分、Barthel指数评分高于对照组(P<0.001)。结论:在高龄不稳定股骨粗隆间骨折治疗中,BHA较PFNA手术时间更长、术中创伤更大(出血量及引流量更高),但术后恢复更快、并发症更少,且髋关节功能及生活能力恢复更优。
Objective:To compare the effect of biological hip arthroplasty(BHA)and proximal femoral nail antirotation(PFNA)in the treatment of elderly patients with unstable intertrochanteric fracture of femur.Methods:An analysis was conducted.From January 2022 to December 2024,70 elderly patients with unstable intertrochanteric fractures of the femur admitted to Dejiang County People's Hospital were divided into the control group(32 cases,PFNA internal fixation)and the observation group(38 cases,BHA)according to the surgical methods.The surgical indicators,incidence of postoperative complications,hip joint function and activities of daily living of the two groups were compared.Results:The operation time of the observation group was longer than that of the control group(P<0.001),the intraoperative blood loss and postoperative drainage volume were higher than those of the control group(P<0.001),and the first time to get out of bed and the weight-bearing time after the operation were shorter than those of the control group(P<0.001).The incidence of postoperative complications in the observation group was lower than that in the control group(P=0.011).At 1,3 and 6 months after the operation,the Harris score and Barthel Index score of the observation group were higher than those of the control group(P<0.001).Conclusion:In the treatment of unstable intertrochanteric fractures in the elderly,BHA has a longer operation time and greater intraoperative trauma(higher blood loss and drainage volume)than PFNA,but it has a faster postoperative recovery,fewer complications,and better recovery of hip joint function and living ability.
