摘要
目的:探讨无肝素维持性血液透析(MHD)结束时应用不停泵密闭式双向回血法的效果。方法:选取2022年7月—2023年7月在肇庆市第一人民医院血液净化中心接受无肝素MHD治疗的患者120例,采用随机数字表法分为对照组(常规密闭式双向回血法)与研究组(不停泵密闭式双向回血法),每组60例。比较两组回输生理盐水量、回血时间。结果:研究组回输生理盐水量低于对照组,回血时间短于对照组,差异有统计学意义(P<0.001);研究组透析器凝血程度低于对照组,差异有统计学意义(P=0.002);两组回血过程中均未发生空气栓塞、心绞痛等并发症。结论:无肝素MHD结束时应用不停泵密闭式双向回血法有一定优势,可减少回输生理盐水量,缩短回血时间,降低透析器凝血风险,且安全性好。
Objective:To investigate the effect of non-stop pump closed bidirectional blood reflux method at the end of heparinfree maintenance hemodialysis(MHD).Methods:A total of 120 patients who received heparin-free MHD treatment at the Blood Purification Center of the First People's Hospital of Zhaoqing from July 2022 to July 2023 were selected.They were divided into a control group(conventional closed bidirectional blood reflux method)and a study group(non-stop pump closed bidirectional blood reflux method)using a random number table,with 60 patients in each group.The amount of saline reinfusion and blood reflux time were compared between the two groups.Results:The amount of saline reinfusion in the study group was lower than that in the control group,and the blood reflux time in the study group was shorter than that in the control group,and the difference was statistically significant(P<0.001).The dialyzer coagulation level in the study group was lower than that in the control group,and the difference was statistically significant(P=0.002).There were no complications such as air embolism and angina pectoris during blood reflux in the two groups.Conclusion:At the end of heparin-free MHD,the application of non-stop pump closed bidirectional blood reflux method has certain advantages,which can reduce the amount of saline reinfusion,shorten the blood reflux time,reduce the risk of dialyzer coagulation,and have good safety.
