摘要
目的比较单孔和三孔胸腔镜肺叶切除术治疗早期非小细胞肺癌(NSCLC)的疗效及对患者术后恢复及远期生存的影响。方法选择88例早期NSCLC患者,采用电脑生成的随机数字表,将患者随机分为2组,单孔胸腔镜肺叶切除组(A组)和三孔胸腔镜肺叶切除组(B组)。比较2组手术情况、术后恢复及远期生存情况。结果2组在手术情况以及所有远期生存指标方面的差异均无统计学意义(P>0.05)。A组术后VAS低于B组、术后住院和引流管拔除时间短于B组,术后引流量少于B组(P<0.05);A组术后FACT-L中情感状态、功能状态、肺癌相关症状的评分高于B组(P<0.05)。结论在早期非小细胞肺癌治疗中,单孔胸腔镜肺叶切除术相比于三孔胸腔镜肺叶切除术,在手术情况和生存情况上未见显著差异,但单孔手术组术后疼痛评分更低、康复时间更短、功能状态更好。
Objective To compare the efficacy of single-port and three-port thoracoscopic lobectomy for early non-small cell lung cancer(NSCLC)and their impact on postoperative recovery and long-term survival of patients.Methods 88 patients with early NSCLC were selected.Using A computer-generated random number table,the patients were randomly divided into 2 groups:the single-port thoracoscopic lobectomy group(Group A)and the three-port thoracoscopic lobectomy group(Group B).The surgical conditions,postoperative recovery and long-term survival of the 2 groups were compared.Results There were no statistically significant differences in surgical conditions and all long-term survival indicators between the 2 groups(P>0.05).The postoperative VAS of group A was lower than that of group B,the postoperative hospital stay and drainage tube removal time were shorter than those of group B,and the postoperative drainage volume was less than that of group B(P<0.05).The scores of emotional state,functional state and lung cancer-related symptoms in FACT-L after surgery in group A were higher than those in group B(P<0.05).Conclusion In the treatment of early non-small cell lung cancer,there is no significant difference in surgical conditions and survival between single-port thoracoscopic lobectomy and three-port thoracoscopic lobectomy.However,the single-port surgery group has a lower postoperative pain score,shorter recovery time,better functional status.
