摘要
胆管内乳头状黏液性肿瘤(Intraductal Papillary Mucinous Neoplasm of the Bile Duct,IPMN-B)是一种具有独特病理特征的胆道系统病变,其核心定义为:起源于胆管上皮细胞的肿瘤样增生性疾病,以导管内黏液异常分泌为病理标志。该病变特征性表现为肿瘤细胞沿胆管壁呈乳头状生长,并持续产生大量黏稠分泌物(黏液或凝胶样物质),这种病理性分泌过程常导致胆道系统黏液潴留,进而引发胆管扩张及梗阻性黄疸等特征性临床表现,既往诊断为分泌黏液的胆管癌中,部分病例可能属于此类疾病。IPMN-B在临床中较为少见,术前诊断存在一定的困难,存在误诊及漏诊的可能。本文通过回顾分析1例胆管乳头状黏液性肿瘤病例的临床资料,以供临床参考。
Intraductal papillary mucinous neoplasm of the bile duct(IPMN-B)is a biliary tract lesion with unique pathological features.Its core definition is a tumor-like proliferative disease originating from the biliary epithelial cells,characterized pathologically by abnormal intraductal mucus secretion.The characteristic manifestation of this lesion is that the tumor cells grow papillarily along the bile duct wall and continuously produce a large amount of thick secretions(mucus or gelatinous substances).This pathological secretion process often leads to mucus retention in the biliary tract system,which in turn causes characteristic bile duct dilatation and obstructive jaundice and other clinical manifestations.In the past,some cases previously diagnosed as mucus-secreting cholangiocarcinoma may belong to this type of disease.This type of disease is relatively rare in clinical practice,and there are certain difficulties in preoperative diagnosis,with the possibility of misdiagnosis and missed diagnosis.This paper reviews and analyze a case of intraductal papillary mucinous neoplasm of the bile duct for clinical reference only.
