摘要
目的探讨舒筋蠲痹汤加温针灸治疗寒湿阻络型肩周炎的临床疗效。方法将符合纳入标准的60例寒湿阻络型肩周炎患者随机分为观察组、对照组,每组30例。基础治疗给予塞来昔布胶囊口服,对照组在基础治疗的基础上加以温针灸治疗,观察组在对照组的基础上加以舒筋蠲痹汤治疗。以中医证候评分、主观疼痛程度McGill疼痛评分、肩关节功能Constant-Murley评分(CMS)及血清前列腺素E2(PGE2)、P物质(SP)水平变化为评价指标。结果治疗后,两组患者的中医证候评分均显著降低,观察组低于对照组(P<0.05)。治疗后,两组患者的McGill评分均显著降低,观察组低于对照组(P<0.05)。治疗后,CMS评分中的疼痛评分明显降低,观察组低于对照组(P<0.05);CMS评分中肩关节活动度评分、日常生活能力评分、肌力评分均明显提升,观察组高于对照组(P<0.05)。治疗后,两组血清PGE2、SP水平均显著降低,观察组低于对照组(P<0.05)。结论对寒湿阻络型肩周炎患者给予舒筋蠲痹汤联合温针灸治疗,可显著减轻肩部疼痛,提升肩关节功能和日常生活能力,降低疼痛因子水平,值得推广。
Objective To explore the clinical efficacy of Shujin Juanbi Decoction combined with warming acupuncture in the treatment of periarthritis of the shoulder caused by cold-dampness blocking the meridians.Methods Sixty patients with cold-dampness blocking the meridians of periarthritis of the shoulder who met the inclusion criteria were randomly divided into the observation group and the control group,with 30 cases in each group.The basic treatment was oral administration of celecoxib capsules.The control group was treated with warm acupuncture on the basis of the basic treatment,and the observation group was treated with Shujin Juanbi Decoction on the basis of the control group.The evaluation indicators were based on the scores of Traditional Chinese Medicine syndrome,the McGill pain score for subjective pain degree,the Constant-Murley score(CMS)for shoulder joint function,and the changes in serum prostaglandin E2(PGE2)and substance P(SP)levels.Results After treatment,the TCM syndrome scores of both groups of patients decreased significantly,and the score of the observation group was lower than that of the control group(P<0.05).After the treatment,the McGill scores of both groups of patients decreased significantly,and the score of the observation group was lower than that of the control group(P<0.05).After the treatment,the pain score in the CMS score decreased significantly,and the score in the observation group was lower than that in the control group(P<0.05).In the CMS score,the scores of shoulder joint range of motion,daily living ability and muscle strength were all significantly improved,and the scores in the observation group were higher than those in the control group(P<0.05).After the treatment,the levels of serum PGE2 and SP in both groups decreased significantly,and those in the observation group were lower than those in the control group(P<0.05).Conclusion For patients with cold-dampness blocking the meridians type of periarthritis of the shoulder,the treatment of Shujin Juanbi Decoction combined with warm acupuncture can significantly relieve shoulder pain,enhance shoulder joint function,improve the ability of daily living,and reduce the level of pain factors.It is worthy of promotion.
