摘要
目的为建立处方审核专业胜任力评价模型以及明确今后培训方向、培训重点以及政策制定提供参考与借鉴。方法结合文献并采用德尔菲法初步提取初始指标,对湖南省14个地市州接受处方审核培训的1050名药师进行问卷调查,采用因子分析构建并验证模型。采用SPSS 26.0进行数据描述统计分析和检验。比较不同级别医疗机构、不同岗位以及不同地区药师的自评得分。结果构建包含3个一级指标(基本知识及职业技能、内驱力、科研教学能力),16个二级指标的评价模型,验证性因子分析结果显示:该模型拟合优度(GFI=0.850,CFI=0.905,NFI=0.898)和信效度(Cronbach’sα系数均大于0.8)良好。三级医疗机构自评分高于一、二级医疗机构(P均<0.01)。人数最庞大的西药调剂药师(n=508,占比48.381%)自评分比临床药师、药品采购岗位、处方审核岗位要低(P均<0.01)。长沙和湘潭药师自评分相对较高,张家界药师自评分相对较低。结论构建医疗机构药师处方审核专业胜任力评价模型具有良好的科学性、适用性。同时,下一步培训重点应进一步扩大各市州一、二级医疗机构、调剂药师的培训范围,全面提升药师处方审核能力。
Objective To evaluate the competence of prescription review and offer insights for future training directions,focuses,and policy making.Methods Initial indicators were collected through literature review and the Delphi method.A questionnaire survey was conducted among 1050 pharmacists from 14 cities and prefectures in Hunan Province who had received prescription review training.Factor analysis was used to validate the model.SPSS 26.0 was used for descriptive statistical analysis and testing.Self-assessment scores of pharmacists from different levels of healthcare institutions,positions,and regions were compared.Results An evaluation model comprising 3 primary indicators(basic knowledge and professional skills,intrinsic motivation,and teaching/research capabilities)and 16 secondary indicators was developed.Confirmatory factor analysis demonstrated good model fit(GFI=0.850,CFI=0.905,and NFI=0.898)and reliability/validity(Cronbach’sαcoefficients all>0.8).Self-assessment scores of pharmacists from tertiary hospitals were higher than those in primary and secondary hospitals(P<0.01).Dispensing pharmacists(n=508,48.381%),the largest subgroup,had lower scores as compared with those of clinical pharmacists,drug procurement staff,and prescription review pharmacists(P<0.01).Pharmacists in Changsha and Xiangtan reported relatively higher self-assessment scores,while those in Zhangjiajie scored lower.Conclusion The established evaluation model for pharmacists’prescription review competence is valid and feasible.Future training efforts should prioritize expanding coverage to primary/secondary healthcare institutions and dispensing pharmacists across all regions.
