基于数字模拟的经骶1骶髂螺钉的初步临床应用

摘要 目的 总结基于数字模拟的经骶1骶髂(S1alar iliac, S1AI)螺钉的临床应用疗效,探讨其可行性及安全性。方法 回顾性分析福建省邵武市立医院自2021年8月至2023年3月采用基于数字模拟的S1AI螺钉固定的11例患者的临床及影像学资料。共使用22枚螺钉,其中10枚为专用髂骨螺钉,12枚为普通万向椎弓根钉。使用原因中,严重骨质疏松、椎弓根螺钉固定效果较差而改用S1AI螺钉4例;由于髂后上棘阻挡、S1椎弓根钉内倾角过小导致螺钉过短而改用S1AI螺钉4例;仅为减少软组织损伤者3例。置钉方法采用我科数字模拟置钉后改良的徒手置钉法。术后复查X线片,CT复查螺钉位置,随访患者症状、内固定松动率、融合率。统计一次置钉成功率、并发症发生率。结果 22枚螺钉一次置钉成功19枚,3枚调整方向后安全置入。术中无血管神经并发症发生。术后X线片及CT复查,穿出率为0。平均随访20.63个月,均融合良好,2例出现螺钉旁松动影。结论 S1AI螺钉置入简单,一次成功率高,固定效果好,能较好地应用于临床。对于骨质疏松无法取得良好固定的患者及髂骨内聚患者,是一项良好的替代方法。
