摘要
目的 分析当前社区卫生服务中心多学科协作的现状并剖析社区医疗的优势,尝试构建多学科医疗合作模式,并提出以家庭医生站点为例的实践路径。方法 通过焦点小组会议、半结构化访谈和文献检索,选取某社区卫生服务站作为研究对象进行调研。结果 本研究构建了以家庭医生站点为例的多学科医疗合作模式,实现多学科专业协同。在该模式中,家庭医生团队作为业务前端,除承担临床诊疗职责外,还承担了居民需求整合、健康全面评估和服务整合的职责。在该医疗模式运行期间,家庭医生签约人数较上年同期由181人增长至409人,并且在探索和实践双中心赋能多学科协作整合式服务模式期间,该站点的总签约居民达到1 305名。结论 家庭医生站点多学科医疗协作模式能够整合多部门资源,提升了家庭医生签约服务的质量和效率。此模式为家庭医生服务深化提供了切实可行的策略,具有推广到更广泛社区卫生服务领域的潜力,为今后类似探索建立了理论和实践基础。
Objective To Analyze the current status of multidisciplinary collaboration in community healthcare service centers and examine the advantageous of community healtheare.Attempt to construct a multidisciplinary healthcare collaboration model and propose a practical path using family doetor stations as an example.Methods Through focus group meetings,semi-structured interviews,and literature search,a community health service station was selected as the research object for investigation.Results This study constructed a multidisciplinary medical cooperation model using family doctor sites as an example to achieve interdisciplinary professional collaboration.In this model,the family doctor team serves as the front-end of the business,responsible not only for clinical diagnosis and treatment,but also for integrating residents'needs;comprehensive health assessment,and service integration.During the operation of the medical model,the number of family doctors signing contracts increased from 181 to 409 compared to the same period last year,and during the exploration and practice of the dual center empowerment of interdiseiplinary collaboration and integrated service model,the total number of signed residents at the site reached 1305.Conclusion The multidisciplinary medical collaboration model of family doctor sites can integrate resources from multiple departments,improving the quality and efficiency of family doetor contract serv ices.This model provides a practical and feasible strategy for deepening family doctor services,with the potential to be promoted to a wider range of community health services,and to establish a theoretical and practical foundation for similar explorations in the future.
