慢阻肺稳定期患者血清miR-4456、CCL3水平与肺功能及预后相关性

Correlation study of serum miR-4456 and CCL3 levels with lung Function and Prognosis in Patients with Stable chronic obstructive pulmonary Disease

摘要 目的探讨慢性阻塞性肺疾病(简称慢阻肺)稳定期患者血清微小RNA-4456(miR-4456)、C-C趋化因子配体3(CCL3)水平及其与肺功能、预后的关系。方法选取本院218例慢阻肺稳定期患者为研究对象,另选取218例同期健康体检者为对照。检测慢阻肺患者血清miR-4456、CCL3水平及肺功能指标;Pearson法分析miR-4456、CCL3与肺功能指标相关性;多因素Logistic回归分析慢阻肺患者预后影响因素;受试者工作特征(ROC)曲线分析miR-4456、CCL3水平对患者预后评估价值。结果与健康志愿者相比,慢阻肺患者血清miR-4456、FEV_(1)、FVC、FEV_(1)/FVC、VE、MVV和PEF较低,CCL3、Raw较高(P<0.05)。miR-4456与CCL3有靶向结合位点,血清miR-4456和CCL3呈负相关(r=-0.461,P=0.001);miR-4456与FEV_(1)、FEV_(1)/FVC、VE、MVV和PEF呈正相关,CCL3与FEV_(1)、FEV_(1)/FVC、VE、MVV和PEF呈负相关,与Raw呈正相关(P<0.05)。miR-4456、CCL3、GOLD分级、PCT、CRP和TNF-α是慢阻肺患者死亡的影响因素(P<0.05)。血清miR-4456、CCL3单独及联合评估慢阻肺患者预后的AUC分别为0.803(0.774~0.854)、0.737(0.673~0.794)、0.858(0.805~0.902),二者联合评估慢阻肺患者预后价值优于单一指标(Z miR-4456-联合=2.624,P=0.009,Z CCL3-联合=3.740,P<0.001)。结论血清miR-4456、CCL3水平与慢阻肺患者肺功能具有相关性,二者联合检测对患者预后评估价值较高。 Objective To explore the levels of serum microRNA-4456(miR-4456)and C-C chemokine ligand 3(CCL3)in patients with stable chronic obstructive pulmonary disease(COPD for short)and their relationships with lung function and prognosis.Methods A total of 218 patients with stable chronic obstructive pulmonary disease in our hospital were selected as the research subjects,and another 218 healthy individuals who underwent physical examinations during the same period were selected as the control group.Detecting the levels of serum miR-4456 and CCL3 and pulmonary function indicators in patients with chronic obstructive pulmonary disease;The correlations between miR-4456,CCL3,and pulmonary function indicators were analyzed by Pearson method;Multivariate Logistic regression analysis was conducted to analyze the prognostic influencing factors of patients with chronic obstructive pulmonary disease.The receiver operating characteristic(ROC)curve was used to analyze the prognostic evaluation value of miR-4456 and CCL3 levels for patients.Results Compared with healthy volunteers,the levels of serum miR-4456,FEV_(1),FVC,FEV_(1)/FVC,VE,MVV,and PEF in patients with chronic obstructive pulmonary disease were lower,while CCL3 and Raw were higher(P<0.05).miR-4456 has targeted binding sites with CCL3,and serum miR-4456 was negatively correlated with CCL3(r=-0.461,P=0.001).miR-4456 was positively correlated with FEV_(1),FEV_(1)/FVC,VE,MVV and PEF.CCL3 was negatively correlated with FEV_(1),FEV_(1)/FVC,VE,MVV,and PEF,and positively correlated with Raw(P<0.05).miR-4456,CCL3,GOLD classification,PCT,CRP,and TNF-αwere the influencing factors of death in patients with chronic obstructive pulmonary disease(P<0.05).The AUCs of serum miR-4456 and CCL3 alone and in combination for evaluating the prognosis of patients with chronic obstructive pulmonary disease were 0.803(0.774-0.854),0.737(0.673-0.794),and 0.858(0.805-0.902),respectively.The prognostic value of the combined evaluation of the two in patients with chronic obstructive pulmonary disease was superior to that of a single indicator(Z miR-4456-combined=2.624,P=0.009,Z CCL3-combined=3.740,P<0.001).Conclusion The levels of serum miR-4456 and CCL3 are correlated with the pulmonary function of patients with COPD,and the combined detection of the two has a high prognostic evaluation value for patients.
