摘要
李聚林教授认为,慢性传输型便秘病位主要是大肠,与肺、脾、胃、肝、肾等脏腑生理功能失调相关。三焦不和,脏腑失调为慢性传输型便秘发生的关键所在,三焦失和为慢性传输型便秘核心病机之一。李教授治疗本病重视梳理三焦,辨证论治:上焦不宣,宜宣通上焦,调畅气机;中焦不和,宜斡旋中焦,调和脾胃;下焦不润,宜温润下焦,滋肾润肠;秘结日久,结肠黑变,宜清毒化瘀,逆转黑变。李教授治疗时注重辨疾病之根本,从根本出发,不一味泻下,避免苦寒泻剂的使用,防止泻剂依赖加重便秘程度,提出通理三焦以治秘的治疗原则,自拟理焦通秘汤通理三焦治疗慢性传输型便秘,还配合饮食、运动、排便习惯的调整等治疗该病。
Professor Li Julin thinks that slow-transit constipation primarily involves the large intestine and is linked to dysfunction of the lung,spleen,stomach,liver,and kidney.The key pathogenesis lies in triple energizer disharmony and visceral dysfunction,with impaired triple energizer coordination being a core mechanism.His treatment emphasizes regulating the triple energizer through syndrome differentiation:For upper energizer obstruction,his treatment focuses on ventilating the upper energizer to regulate qi movement.For middle energizer imbalance,harmonizing the middle energizer to restore spleen-stomach function.For lower energizer desiccation,warming and moistening the lower energizer to nourish kidneys and intestines.For prolonged constipation with melanosis coli,detoxifying and resolving stasis to reverse mucosal pigmentation.Professor Li prioritizes addressing root causes over purgation,avoiding harsh cathartics to prevent dependency.He advocates"regulating the triple energizer to relieve constipation"as the guiding principle,prescribing the self-formulated Energizer-Regulating and Constipation-Dredging Decoction,complemented by dietary adjustments,exercise,and bowel habit training,etc.
