摘要
目的探讨血府逐瘀汤联合针刺在气滞血瘀型腰椎间盘突出症(LDH)患者中的应用效果。方法选取2023年7月—2024年12月略阳县中医医院收治的120例气滞血瘀型LDH患者为研究对象,按随机数字表法将其分为对照组和观察组,各60例。对照组采用针刺治疗,观察组在对照组基础上加用血府逐瘀汤治疗。对比两组的疼痛程度、中医证候积分、腰椎功能、生活质量及不良反应发生情况。结果治疗后,观察组疼痛视觉模拟评分及中医证候积分均低于对照组,组间差异有统计学意义(P<0.05);观察组腰椎日本骨科协会评估治疗分数高于对照组,Oswestry功能障碍指数评分低于对照组,组间差异有统计学意义(P<0.05);观察组健康调查简表中情感职能、社会功能、生命活力、总体健康、精神健康、生理功能、生理职能、躯体疼痛评分均高于对照组,组间差异有统计学意义(P<0.05)。两组治疗期间均未发生不良反应。结论血府逐瘀汤联合针刺治疗气滞血瘀型LDH患者可缓解疼痛程度,减轻腰椎功能障碍,提升生活质量,且安全有效。
Objective To explore the application effect of Xuefu Zhuyu decoction combined with acupuncture in patients with lumbar disc herniation(LDH)of qi stagnation and blood stasis type.Methods A total of 120 patients with LDH of qi stagnation and blood stasis type admitted to Lueyang County Hospital of Traditional Chinese Medicine from July 2023 to December 2024 were selected as the research objects.According to the random number table method,they were divided into a control group and an observation group,with 60 cases in each group.The control group was treated with acupuncture,and the observation group was treated with Xuefu Zhuyu decoction on the basis of the control group.The pain degree,Traditional Chinese Medicine syndrome score,lumbar spine function,quality of life and occurrence of adverse reactions were compared between the two groups.Results After treatment,the pain Visual Analogue Scale score and Traditional Chinese Medicine syndrome score of the observation group were lower than those of the control group,and the differences between the two groups were statistically significant(P<0.05);the Japanese Orthopaedic Association score of lumbar spine in the observation group was higher than that in the control group,and the Oswestry Disability Index score was lower than that in the control group,and the differences between the two groups were statistically significant(P<0.05).The scores of emotional function,social function,vitality,general health,mental health,physical function,physical function and body pain in the 36-item Short-Form health survey in the observation group were higher than those in the control group,and the differences between the two groups were statistically significant(P<0.05).No adverse reactions occurred in the two groups during treatment.Conclusion Xuefu Zhuyu decoction combined with acupuncture in the treatment of LDH patients with qi stagnation and blood stasis can relieve pain,reduce lumbar spine dysfunction,and improve the quality of life,which is safe and effective.
