摘要
目的分析乳腺影像报告数据系统(BI-RADS)3、4类结节鉴别诊断中剪切波弹性成像(SWE)联合动态增强磁共振(DCE-MRI)检查的应用价值。方法回顾性选择女性乳腺结节患者107例,所有患者均行SWE、DCE-MRI检查,以病理结果为金标准将患者分为良性组45例,恶性组62例。比较各组SWE弹性模量值及DCE-MRI动力学参数,分析不同检查方法对BI-RADS 3、4类结节的诊断效能。结果恶性结节的E_(max)、E_(mean)、E_(sd)、K_(trans)、K_(ep)水平高于良性结节,E_(min)水平低于良性结节(P<0.05)。SWE、DCE-MRI联合方案在阳性和阴性病例识别中均表现最优。SWE、DCE-MRI联合检查对乳腺BI-RADS 3、4类结节的诊断准确度、特异度、灵敏度、阳性预测值及阴性预测值均高于SWE、DCE-MRI单独检查(P<0.05)。结论乳腺BI-RADS 3、4类结节鉴别诊断中SWE联合DCE-MRI检查有着较高的应用价值,可较好地指导临床治疗。
Aim This study aims to analyze the diagnostic value of shear wave elastography(SWE)combined with dynamic contrast-enhanced magnetic resonance imaging(DCE-MRI)in differentiating breast imaging reporting and data system(BI-RADS)category 3 and 4 nodules.Methods A total of 107 female patients with breast nodules were retrospectively selected.All patients underwent SWE and DCE-MRI examinations.Using pathological results as the golden standard,patients were divided into a benign group(45 cases)and a malignant group(62 cases).The SWE elastic modulus values and DCE-MRI kinetic parameters were compared between groups,and the diagnostic performance of different methods for BI-RADS category 3 and 4 nodules was analyzed.Results Malignant nodules exhibited higher E_(max),E_(mean),E_(sd),K_(trans),and K_(ep)levels but lower E min levels compared to benign nodules(P<0.05).The combined SWE and DCE-MRI approach demonstrated optimal performance in identifying both positive and negative cases.The combined approach achieved significantly higher diagnostic accuracy,specificity,sensitivity,positive predictive value,and negative predictive value compared to SWE or DCE-MRI alone(P<0.05).Conclusion SWE combined with DCE-MRI holds high clinical value in differentiating breast BI-RADS category 3 and 4 nodules,providing reliable guidance for clinical management.
