摘要
目的 探讨老年患者住院期间发生下肢深静脉血栓(DVT)的危险因素。方法 收集2022年6月至2024年5月首都医科大学附属北京天坛医院收治的196例老年患者的临床资料,按照住院期间是否发生下肢DVT将其分为DVT组(n=29)与对照组(n=167)。收集所有患者入院时的临床特征、住院诊治经过的主要指标以及住院期间下肢DVT的发生情况,分析老年患者住院期间发生下肢DVT的危险因素。结果 DVT组患者中下肢残疾、痴呆、心力衰竭、慢性阻塞性肺疾病、既往静脉血栓栓塞(VTE)病史比例均高于对照组患者,差异均有统计学意义(P﹤0.05)。多因素分析结果显示,下肢残疾、痴呆、心力衰竭、慢性阻塞性肺疾病以及既往VTE病史均为老年患者住院期间发生下肢DVT的独立危险因素(P﹤0.05)。结论 老年患者住院期间具有较高的下肢DVT发生风险,下肢残疾、痴呆、心力衰竭、慢性阻塞性肺疾病、既往VTE病史均是其危险因素,临床可以适当参考,从而及时识别高危患者。
Objective To explore the risk factor for deep vein thrombosis(DVT)of the lower extremity in elderly inpatients during hospitalization.Method Clinical data of 196 elderly inpatients admitted to Beijing Tiantan Hospital,Capital Medical University from June 2022 to May 2024 were collected.They were divided into the DVT group(n=29)and the control group(n=167)based on whether lower extremity DVT occurred during hospitalization.Clinical characteristics at admission,main indicators of hospitalization and treatment,and the occurrence of lower extremity DVT during hospitalization of all patients were collected.Risk factors for lower extremity DVT in elderly inpatients were analyzed.Result The proportions of lower extremity disability,dementia,heart failure,chronic obstructive pulmonary disease and previous venous thromboembolism(VTE)history in the DVT group were all higher than those in the control group,the differences were statistically significant(P<0.05).The results of multivariate analysis showed that lower extremity disability,dementia,heart failure,chronic obstructive pulmonary disease and previous VTE history were all independent risk factors for lower extremity DVT in elderly inpatients(P<0.05).Conclusion Elderly inpatients during hospitalization have the higher risk of developing lower extremity DVT,lower extremity disability,dementia,heart failure,chronic obstructive pulmonary disease,and previous history of VTE are all risk factors.Clinicians can appropriately refer to these factors to promptly identify high-risk patients.
