摘要
慢加急性肝衰竭(acute-on-chronic liver failure,ACLF)是在慢性肝病基础上,短期内出现肝功能急性失代偿的临床综合征,其特征是常伴发多器官功能衰竭,短期病死率高。由于ACLF的病情重、预后差,故准确、快速识别该类疾病,并合理化治疗,对改善预后具有重要意义。然而,由于东西方病因及流行病学的差异,目前关于ACLF的诊断标准缺乏一致的定义,这使得ACLF患者的治疗时机、疾病管理、预后评估以及不同地区间患病人群的比较缺乏同质性。因此将不同研究的定义统一起来,建议一个广泛适用的ACLF定义已成为近年来研究热点。关于ACLF的概念一直在不断更新和完善,本文旨在对ACLF定义的出现、变化和完善以及不同定义间的差异进行综述。
Acute-on-chronic liver failure(ACLF)is a clinical syndrome characterized by acute decompensation of liver function in a short period of time based on chronic liver disease,which is often accompanied by multi-organ failure and has a high short-term mortality rate.Due to the severity of ACLF and poor prognosis,it is of great significance to accurately and quickly identify this type of disease and rationalize its treatment to improve the prognosis.However,due to the differences in etiology and epidemiology between the East and the West,there is a lack of consistent definition of the diagnostic criteria for ACLF,which makes the timing of treatment,disease management,prognostic assessment,and the comparison of patients with ACLF between different regions lack homogeneity.Therefore,it has become a hot topic in recent years to unify the definitions of different studies and suggest a widely applicable definition of ACLF.The concept of ACLF has been constantly updated and improved,and this article aimed to review the emergence,variation and improvement of the definition of ACLF,as well as the differences between different definitions.
