摘要
目的评估高频振荡通气联合容量保证治疗早产儿呼吸窘迫综合征(Respiratory distress syndrome,RDS)的有效性及安全性。方法回顾性分析2018年1月—2023年12月本院收治的采用高频振荡通气(High-frequency oscillation ventilation,HFOV)治疗的胎龄<32周早产儿RDS病例共59例,其中28例采用HFOV+容量保证(Volume guarantee,VG),31例单一HFOV通气。结果HFOV+VG组早产儿通气期间VThf波动小于HFOV组,低碳酸血症及高碳酸血症发生率低于单一HFOV组,总用氧时间少于单一HFOV组,差异具有统计学意义(P<0.05);两组机械通气时间,及颅内出血、坏死性小肠结肠炎、脓毒血症、早产儿视网膜病、气漏综合征发生率比较,差异无统计学意义(P>0.05)。结论对比单一HFOV,HFOV+VG减少了VThf的波动及早产儿发生低碳酸血症及高碳酸血症的风险,缩短了早产儿用氧时间,在早产儿RDS治疗中有很好的应用前景,但需进一步前瞻性研究评估有效性及安全性。
Objective To assess the efficacy and safety of high-frequency oscillatory ventilation(HFOV)combined with volume guarantee(VG)in the treatment of preterm infants with respiratory distress syndrome(RDS).Methods A retrospective analysis was performed on 59 preterm infants with RDS(gestational age<32 weeks)who received rescue treatment with HFOV in our hospital from January 2018 to December 2023.Among them,28 were treated with HFOV+VG,and 31 were treated with HFOV alonee.Results The fluctuation of VThf during the ventilation period in the HFOV+VG group was less than that in the HFOV group,and the incidence of hypocapnia and hypercapnia in preterm infants in the HFOV+VG group was significantly lower.The duration of oxygen inhalation in the HFOV+VG group was shorter than that in the HFOV group(all P<0.05).There was no significant difference in the duration of mechanical ventilation,the occurrence of intracranial hemorrhage,necrotizing enterocolitis,sepsis,retinopathy of prematurity,and air leak syndrome between the two groups(P>0.05).Conclusions Compared with HFOV alone,HFOV+VG reduces the fluctuation of VThf and the risk of hypocapnia and hypercapnia in preterm infants,and shortens the duration of oxygen inhalation.It has promising application prospects in the treatment of RDS in preterm infants,but further prospective studies are needed to assess its efficacy and safety.
