摘要
目的探讨短肽型联合支链氨基酸型肠内营养方案对肝硬化腹水患者肝功能、营养状况和胃肠道耐受的影响。方法选取2021年6月至2024年6月福建省泉州市第一医院感染病科收治的92例肝硬化腹水患者,随机分为两组,在常规饮食干预基础上,对照组接受整蛋白型肠内营养干预,观察组接受短肽型联合支链氨基酸型肠内营养干预,比较两组患者肝功能、营养状况及胃肠道耐受情况。结果干预后,观察组血清丙氨酸氨基转移酶、天冬氨酸氨基转移酶、总胆红素、凝血酶原时间及血浆氨水平,营养风险筛查2002评分及胃肠道不良反应总发生率均低于对照组,差异有统计学意义(P<0.05);干预后,观察组血清总蛋白、白蛋白、前白蛋白及转铁蛋白均高于对照组,差异有统计学意义(P<0.05)。结论短肽型联合支链氨基酸型肠内营养方案可改善肝硬化腹水患者的肝功能,营养状况和胃肠道耐受性。
Objective To explore the effects of short peptide combined with branched chain amino acid enteral nutrition program on liver function,nutritional status and gastrointestinal tolerance in patients with cirrhotic ascites.Methods A total of 92 patients with cirrhotic ascites admitted to the Department of Infectious Disease of the Quanzhou First Hospital of Fujian Province from June 2021 to June 2024 were selected and randomly divided into two groups.On the basis of conventional dietary intervention,the control group received whole protein enteral nutrition intervention,and the observation group received short peptide combined with branched chain amino acid enteral nutrition intervention,and the liver function,nutritional status and gastrointestinal tolerance of patients in the two groups were compared.Results After the intervention,the serum alanine aminotransferase,aspartate aminotransferase,total bilirubin,prothrombin time and plasma ammonia levels,nutritional risk screening 2002 scores and the total incidence of gastrointestinal adverse reactions of the observation group were lower than those of the control group,and the difference was statistically significant(P<0.05),after the intervention,the serum total protein,albumin,pre albumin and transferrin were higher than those of the control group,and the difference was statistically significant(P<0.05).Conclusion Short peptide combined with branched chain amino acid enteral nutrition program can improve liver function,nutritional status and gastrointestinal tolerance of patients with cirrhotic ascites.
