摘要
目的:探究血清热休克蛋白90α(HSP90α)、磷脂酰肌醇蛋白多糖-3(GPC3)、甲胎蛋白异质体3(AFP-L3)水平对早期原发性肝癌(PHC)的诊断价值。方法:选取PHC患者51例为PHC组,肝硬化患者51例为肝硬化组,并选取同期体检健康者51例为健康对照组。比较三组血清HSP90α、GPC3、AFP-L3水平及肝功能差异。分析血清HSP90α、GPC3、AFP-L3水平与肝功能的相关性、PHC影响因素以及三者对早期PHC的诊断价值。结果:PHC组血清HSP90α、GPC3、AFP-L3、血清谷草转氨酶(AST)、谷丙转氨酶(ALT)、碱性磷酸酶(ALP)水平高于肝硬化组和健康对照组,且肝硬化组高于健康对照组(均P<0.05)。血清HSP90α、GPC3、AFP-L3水平与AST、ALT、ALP水平呈正相关(均P<0.05)。Logistic回归结果显示,血清HSP90α、GPC3、AFP-L3水平高是PHC的独立危险因素(均P<0.05)。ROC曲线分析显示,血清HSP90α、GPC3、AFP-L3水平对早期PHC均有一定诊断价值,三项联合的诊断价值高于单项(均P<0.05)。结论:PHC患者血清HSP90α、GPC3、AFP-L3水平升高,与肝功能存在负相关,三项联合诊断早期PHC的价值较高。
Objective:To Investigate the diagnostic value of serum heat shock protein 90α(HSP90α),glypican-3(GPC3),and lentil lectin-reactive alpha-fetoprotein-L3(AFP-L3)levels in early primary hepatocellular carcinoma(PHC).Methods:Fifty-one patients with PHC were selected as the PHC group,51 patients with liver cirrhosis were selected as the liver cirrhosis group,and 51 healthy people were selected as the healthy control group during the same period.The serum levels of HSP90α,GPC3,AFP-L3 and liver function were compared among the three groups.The correlation between serum HSP90α,GPC3,AFP-L3 levels and liver function,the influencing factors of PHC,and the diagnostic value of the three in early PHC were analyzed.Results:The PHC group had significantly higher serum levels of HSP90α,GPC3,AFP-L3,AST,ALT,and ALP than those of the liver cirrhosis group and the healthy control group,and the liver cirrhosis group had significantly higher serum levels of HSP90α,GPC3,and AFP-L3 than those of the healthy control group(all P<0.05).The levels of serum HSP90α,GPC3 and AFP-L3 were positively correlated with the levels of AST,ALT and ALP(all P<0.05).Logistic regression analysis showed that serum HSP90α,GPC3 and AFP-L3 levels were independent risk factors for PHC(all P<0.05).ROC curve analysis showed that serum HSP90α,GPC3 and AFP-L3 levels had certain diagnostic value for early PHC,and the diagnostic value of the combination of the three was higher than that of each single item(all P<0.05).Conclusion:The serum levels of HSP90α,GPC3 and AFP-L3 in patients with PHC are increased,and negatively correlated with liver function.The combination of the three has a higher diagnostic value for early PHC.
