摘要
目的:探讨腺病毒肺炎患儿外周血血管内皮生长因子(VEGF)、可溶性髓细胞触发受体-1(sTREM-1)及分泌型磷脂酶A2(sPLA2)水平变化及临床意义。方法:选取腺病毒肺炎患儿102例为观察组,其中轻症患儿68例,重症患儿34例。选取健康体检儿童100例为对照组。检测外周血VEGF、sTREM-1、sPLA2水平及肺功能指标[每千克体重潮气量(Vt/kg)和潮气呼气峰流速(PTEF)]。比较观察组与对照组外周血VEGF、sTREM-1及sPLA2水平差异,以及轻症与重症患儿VEGF、sTREM-1及sPLA2水平及肺功能指标差异;分析外周血VEGF、sTREM-1、sPLA2水平与肺功能指标的相关性,以及外周血VEGF、sTREM-1、sPLA2预测重症腺病毒肺炎的价值。结果:观察组VEGF、sTREM-1和sPLA2水平高于对照组(均P<0.05)。观察组重症患儿VEGF、sTREM-1、sPLA2水平及PTEF高于轻症患儿,Vt/kg低于轻症患儿(均P<0.05)。外周血VEGF、sTREM-1、sPLA2水平与Vt/kg呈负相关,与PTEF呈正相关(均P<0.05)。外周血VEGF、sTREM-1、sPLA2水平均对预测重症腺病毒肺炎有一定价值,其中sPLA2水平在预测重症腺病毒肺炎中价值最高(均P<0.05)。结论:腺病毒肺炎患儿外周血VEGF、sTREM-1及sPLA2水平升高,与患儿肺功能及疾病严重程度密切相关,在预测重症腺病毒肺炎方面具有应用价值,其中sPLA2价值较高。
Objective:To study the changes in the levels of vascular endothelial growth factor(VEGF),soluble triggering receptor expressed on myeloid cells-1(sTREM-1),and secretory phospholipase A2(sPLA2)in peripheral blood of children with adenovirus pneumonia and their clinical significance.Methods:A total of 102 children with adenovirus pneumonia were enrolled as the observation group,including 68 mild children and 34 severe children.A total of 100 healthy children were selected as the control group.The levels of VEGF,sTREM-1 and sPLA2 in peripheral blood and pulmonary function indexes(Vt/kg and PTEF)were detected.The levels of VEGF,sTREM-1,and sPLA2 in peripheral blood were compared between the observation group and the control group,and the levels of VEGF,sTREM-1,and sPLA2 and pulmonary function indexes were compared between the mild and severe children.The correlation between peripheral blood VEGF,sTREM-1,sPLA2 levels and pulmonary function indexes was analyzed,and the value of peripheral blood VEGF,sTREM-1,sPLA2 in predicting severe adenovirus pneumonia was analyzed.Results:The levels of VEGF,sTREM-1,and sPLA2 in the observation group were higher than those in the control group(all P<0.05).The levels of VEGF,sTREM-1,sPLA2 and PTEF in the severe group were higher than those in the mild group,and Vt/kg was lower than that in the mild group(all P<0.05).The levels of VEGF,sTREM-1 and sPLA2 in peripheral blood were negatively correlated with Vt/kg and positively correlated with PTEF(all P<0.05).The levels of VEGF,sTREM-1 and sPLA2 in peripheral blood all had certain value in predicting severe adenovirus pneumonia,and sPLA2 level had the highest value in predicting severe adenovirus pneumonia(all P<0.05).Conclusion:The levels of VEGF,sTREM-1,and sPLA2 in peripheral blood are increased in children with adenovirus pneumonia,which are closely related to lung function and disease severity.VEGF,sTREM-1,and sPLA2 have application values in predicting severe adenovirus pneumonia,and splA2 has a higher value.
