摘要
目的旨在通过双能X线骨密度仪(dual-energy X-ray absorptiometry,DXA)检测青少年脑性瘫痪(cerebral palsy,CP)患者的腰椎骨密度,探讨不同瘫痪程度与腰椎骨密度的关联性,以了解其骨发育状况。方法选取2021年1月—2024年1月在广州市社会福利院康复医院收治的105例10~18岁青少年CP患者,根据瘫痪程度分为轻度脑性瘫痪组(n=36)、中度脑性瘫痪组(n=34)和重度脑性瘫痪组(n=35)。另选取32名运动功能正常的同年龄段青少年作为运动功能正常组。对各组间低骨量发生率和腰椎骨密度进行统计学比较分析。结果低骨量发生率在运动功能正常组、轻度脑性瘫痪组、中度脑性瘫痪组和重度脑性瘫痪中分别为23.5%、30.8%、78.6%和77.3%。各组间腰椎骨密度差异有统计学意义(P<0.05)。运动功能正常组(0.723±0.148)g/cm^(2)的腰椎骨密度高于轻度脑性瘫痪组的(0.579±0.118)g/cm^(2)、中度脑性瘫痪组的(0.529±0.077)g/cm^(2)和重度脑性瘫痪组的(0.467±0.089)g/cm^(2),差异均有统计学意义(P<0.05)。轻、中度脑性瘫痪组的腰椎骨密度高于重度脑性瘫痪组,差异有统计学意义(P<0.05),而轻度与中度脑性瘫痪组的腰椎骨密度差异无统计学意义(P>0.05)。结论青少年CP患者的腰椎骨密度随瘫痪程度的加重而逐渐降低,提示临床应加强对该群体骨质疏松的早期防治。
Objective To measure the lumbar bone mineral density in adolescent cerebral palsy(CP)patients using dual-energy X-ray absorptiometry(DXA),and to investigate the association between different degrees of paralysis and lumbar bone mineral density,thereby understanding their bone development status.Methods A total of 105 adolescent cerebral palsy patients aged 10-18 years,admitted to Rehabilitation Hospital,Guangzhou Social Welfare Institute from January 2021 to January 2024,were randomly selected.They were categorized into mild cerebral palsy group(n=36),moderate cerebral palsy group(n=34),and severe cerebral palsy group(n=35)based on their degree of paralysis.Additionally,32 adolescents of the same age range with normal motor function were selected as the normal motor function group.Statistical comparative analysis of low bone mass incidence and lumbar bone mineral density was conducted among all groups.Results The incidence of low bone mass was 23.5%,30.8%,78.6%,and 77.3%in the normal motor function group,mild cerebral palsy group,moderate cerebral palsy group,and severe cerebral palsy group,respectively.There were statistically significant differences in lumbar bone mineral density among all groups(P<0.05).The lumbar bone mineral density of the normal motor function group(0.723±0.148 g/cm^(2))was higher than that of the mild cerebral palsy group(0.579±0.118 g/cm^(2)),the moderate cerebral palsy group(0.529±0.077 g/cm^(2)),and the severe cerebral palsy group(0.467±0.089 g/cm^(2)),and the differences were statisticlly significant(P<0.05).Furthermore,the lumbar bone mineral density of the mild and moderate cerebral palsy groups was significantly higher than that of the severe cerebral palsy group(P<0.05),while no statistically significant difference was found between the mild and moderate cerebral palsy groups(P>0.05).Conclusion The lumbar bone mineral density of adolescent cerebral palsy patients progressively decreases as the severity of paralysis increases,suggesting the need for enhanced early prevention and treatment of osteoporosis in this population in clinical settings.
