从“水郁折之”论治小儿肾病

摘要 本文结合易经卦象,利用象数思维,基于“水郁折之”理论探讨小儿肾病(NS)的病因病机与治法,从巽卦、乾卦、坤卦角度讨论病因,认为发病原因与先天禀赋和后天环境密不可分,病位与肺脾肾密切相关,病机关键在肾失闭藏,精微物质外泄,水邪停聚。多种因素如风气、湿气、热气、瘀血以及体虚等相互影响并逐步演变,导致肾脏的水液代谢功能受阻。临床治疗方面,以风水涣卦、水泽节卦、水火未济卦、天地否卦、泽地萃卦、地火明夷卦、泽水困卦、天泽履卦共八大证型,立足病机,分析卦象,以“折之”立法,提出了宣折、通折、清折、化折、健折、温折、滋折、补折八种治法,为小儿NS的中医治疗开阔了思路。 In this paper,the etiology,pathogenesis and treatment of nephropathy in children were discussed from the perspectives of Xun gua,Qian gua and Kun Gua.It was thought that the source of the illness was linked to innate characteristics and acquired surroundings,with the lung,spleen,and kidney being the most proximate sites.The key to the pathogenesis was the kidney’s lack of storage,the leakage of essence,and the cessation of water and fluid accumulation.Wind,dampness,heat,blood stasis and deficiency affect each other and evolve to cause kidney qi stagnation.In terms of treatment,Feng-huan Gua,Shuize-Jie Gua,Shui-Huo Wei-ji Gua,Tiandan-Fu Gua,Ze-Di Gua,Dihuo Mingyi Gua,Ze-Shui Tuan,Tianze Luoguan Gua,based on the pathogenesis of disease,analysis of the guA,and“folding”legislation,this article proposed eight kinds of treatment methods for promoting folding,smoothing folding,clearing folding,reducing folding,healthy folding,warm folding,Zi folding,and supplementation folding,which opened up ideas for the Chinese medicine treatment of children with nephropathy.
