摘要
糖尿病肾病(DKD)是由糖尿病发展而来的慢性肾脏病,随着糖尿病患病群体的不断扩大,其发病率呈逐渐上升趋势。“阳化气,阴成形”的观点首见于《黄帝内经》,基于此理论,我们认为DKD病因关键在于“虚、痰、瘀、浊、毒”互结,“阳化气不足,阴成形太过”为其主要病机特点。在DKD治疗方面,本文从脏腑关系角度入手,提出以肾立极,从肺、脾、肾论治,兼顾心、肝,调畅三焦;治疗方法上以温阳益气为治疗大法,同时结合活血化瘀法、涤痰消癥法、化浊排毒法,旨在为DKD的中医治疗开拓新思路。
Diabetic kidney disease(DKD)is a chronic kidney disease developed from diabetes mellitus,and its incidence is increasing year by year.The view of“Yang transforming qi and Yin forming shape”was first found in Huangdi Neijing.Based on this theory,we believe that the key cause of DKD is the interaction of deficiency,phlegm,blood stasis,turbidity and poison,and its main pathogenesis is characterized by insufficient Yang transforming qi and Yin forming shape.In the treatment of DKD,from the perspective of the relationship between zang-fu viscera,this paper proposes that the kidney should be the centre,the lung,spleen and kidney should be treated,the heart and liver should be taken into account,and regulating chang sanjiao in the meantime.In the treatment method,warming Yang and supplementing qi are used as the treatment method,and at the same time,the method of promoting blood circulation and removing blood stasis,the method of clearing phlegm and eliminating lump,and the method of removing turbidity and toxins are combined,so as to open up new ideas for TCM treatment of DKD.
