摘要
目的:探讨健脾益肾化浊方联合二甲双胍治疗多囊卵巢综合征(PCOS)合并胰岛素抵抗(IR)的临床疗效及对患者性激素和糖脂代谢的影响。方法:选取106例PCOS-IR患者随机分为观察组(健脾益肾化浊方+二甲双胍,n=53)和对照组(二甲双胍单药,n=53),疗程3个月经周期。评估两组治疗前后基础体温(BBT)排卵率、性激素水平[总睾酮(T)、黄体生成素(LH)、卵泡刺激素(FSH)]、糖脂代谢指标[空腹血糖(FPG)、空腹胰岛素(FINS)、HOMA-IR、血脂谱及胰岛素敏感性(葡萄糖输注率GIR、M值)],并监测安全性。结果:治疗后,观察组BBT排卵率显著高于对照组(P<0.05);两组性激素(T、LH)水平均显著降低(P<0.05),但组间比较无统计学差异(P>0.05)。糖脂代谢方面,观察组FINS、HOMA-IR、总胆固醇(TC)、三酰甘油(TG)改善幅度均优于对照组(均P<0.05),胰岛素敏感性指标(GIR、M值)提升更显著(P<0.001)。两组不良反应发生率比较无统计学差异(P>0.05)。结论:健脾益肾化浊方联合二甲双胍可协同改善PCOS-IR患者排卵功能及糖脂代谢紊乱,疗效优于二甲双胍单药治疗,且安全性良好。
Objective:To investigate the clinical efficacy of Jianpi Yishen Huazhuo formula combined with metformin in treating polycystic ovary syndrome(PCOS)complicated by insulin resistance(IR)and its effects on sex hormones and glucose-lipid metabolism.Methods:A total of 106 PCOS-IR patients were randomly divided into an observation group(Jianpi Yishen Huazhuo formula+metformin,n=53)and a control group(metformin alone,n=53)for a treatment course of three menstrual cycles.Outcomes included basal body temperature(BBT)ovulation rate,sex hormone levels[total testosterone(T),luteinizing hormone(LH),follicle-stimulating hormone(FSH)],glucose-lipid metabolism indicators[fasting plasma glucose(FPG),fasting insulin(FINS),HOMA-IR,lipid profiles],insulin sensitivity[glucose infusion rate(GIR),M-value],and safety monitoring.Results:After treatment,the BBT ovulation rate in the observation group was significantly higher than in the control group(P<0.05).Both groups showed significant reductions in sex hormone levels(T,LH;P<0.05),with no intergroup differences(P>0.05).In glucose-lipid metabolism,the observation group demonstrated superior improvements in FINS,HOMA-IR,total cholesterol(TC),and triglycerides(TG)compared to the control group(P<0.05).Insulin sensitivity indices(GIR,M-value)were also significantly enhanced in the observation group(P<0.001).No statistical difference in adverse reaction rates was observed between the groups(P>0.05).Conclusion:The combination of Jianpi Yishen Huazhuo formula and metformin synergistically improves ovulation function and glucose-lipid metabolism in PCOS-IR patients,with superior efficacy to metformin monotherapy and good safety.
