摘要
目的 探讨单侧双通道内镜术治疗腰椎间盘突出合并腰椎管狭窄的效果。方法 对2021年至2024年收治的120例腰椎间盘突出合并腰椎管狭窄患者的临床资料进行回顾性分析,其中60例采用经皮椎间孔镜术治疗,设为对照组,另外60例采用单侧双通道内镜术治疗,设为观察组。记录并比较两组患者疗效、术中出血量、手术时间、住院时间、Oswestry功能障碍指数(ODI)、日本骨科学会(JOA)量表、视觉疼痛模拟评分(VAS)表、肿瘤坏死因子-α(TNF-α)、白细胞介素-4(IL-7)及C反应蛋白(CRP)水平。结果 观察组治疗有效率高于对照组(P<0.05)。观察组术中出血量及手术时间水平均低于对照组(P<0.05)。术后3个月,观察组ODI及VAS水平均低于对照组,JOA水平高于对照组(P<0.05)。术后2 d,观察组TNF-α、IL-7及CRP水平均低于对照组(P<0.05)。结论 采用单侧双通道内镜术治疗对腰椎间盘突出合并腰椎管狭窄的疗效确切,较利于降低患者术后疼痛及炎症反应程度,对患者腰椎功能改善效果明显。
Objective To investigate the efficacy of unilateral dual-channel endoscopy as a treatment for lumbar disc herniation in conjunction with lumbar spinal stenosis.Methods The data of 120 patients with lumbar disc herniation and spinal stenosis admitted to our hospital from 2021 to 2024 were analyzed.60 cases were treated with percutaneous intervertebral foramenoscopy and the other 60 with unilateral dual-channel endoscopy.The efficacy,intraoperative bleeding,operative time,hospital stay,Oswestry dysfunction index(ODI),Japanese Orthopaedic Association(JOA)scale,visual pain analog score(VAS)scale,tumor necrosis factor-alpha(TNF-α),interleukin-4(IL-7),and C-reactive protein(CRP)levels of the two groups were recorded and compared.Results The observed treatment group exhibited a higher effectiveness rate than the control group(P<0.05).The levels of intraoperative bleeding and operative time in the observation group were lower than those in the control group(P<0.05).Three months after the surgical procedure,the levels of ODI and VAS in the observation group were lower than those in the control group,while the level of JOA was higher in the control group(P<0.05).At the two-day postoperative mark,the levels of TNF-α,IL-7 and CRP in the observation cohort were demonstrably reduced in comparison to their counterparts in the control group(P<0.05).Conclusion The efficacy of unilateral dual-channel endoscopic treatment for lumbar disc herniation combined with lumbar spinal stenosis is precise,more conducive to reducing the degree of postoperative pain and inflammatory reaction of the patient,and the improvement of the patient's lumbar spine function is obvious.
