颈前路零切迹融合器在颈椎病治疗中的临床观察

摘要 目的 探究显微镜下颈前路零切迹融合器(Zero-P)在单节段脊髓型颈椎病中的应用效果。方法 回顾性分析2021年6月至2023年6月本院收治的68例行显微镜下前路颈椎间盘切除融合术(ACDF)治疗单节段脊髓型颈椎病患者的临床资料,其中使用颈前路零切迹融合器(Zero-P组)29例,使用钛板(钛板组)39例。比较两组脊髓受压距手术减压时间、手术时间、术中出血量,术前术后脊髓功能、疼痛程度、颈椎曲度、椎间隙高度,以及术后并发症发生率。结果 Zero-P组手术时间少于钛板组(P<0.05),术中出血量两组差异无统计学意义(P>0.05);两组患者术后JOA评分、VAS评分、Cobb角及椎间隙高度均较术前改善(P<0.05),但两组改善率差异无统计学意义(P>0.05);Zero-P组的术后吞咽困难发生率低于钛板组(P<0.05)。结论 显微镜下颈前路零切迹融合器在单节段脊髓型颈椎病手术中较钛板具有手术时间短,吞咽困难发生率低等优点。 Objective To investigate the application effect of the zero-profile interbody fusion device(Zero-P)under microscope in single segment cervical spondylotic myelopathy(CSM).Methods A retrospective analysis was conducted on the clinical data of 68 patients with single segment CSM treated with anterior cervical discectomy and fusion(ACDF)under microscope from June 2021 to June 2023.Among them,29 cases were treated with anterior cervical Zero-P(Zero-P group)and 39 cases were treated with titanium plate(titanium plate group).The surgical time,intraoperative blood loss,preoperative and postoperative spinal cord function,pain level,cervical curvature,intervertebral height,and incidence of postoperative complications were compared between two groups.Results The Zero-P group had a shorter surgical time than the titanium plate group(P<0.05),and there was no statistically significant difference in intraoperative bleeding between the two groups(P>0.05).The postoperative JOA score,VAS score,Cobb angle,and intervertebral height of both groups of patients improved compared to preoperative levels(P<0.05),but the difference in improvement rate between the two groups was not statistically significant(P>0.05).The incidence of postoperative dysphagia in the Zero-P group was lower than that in the titanium plate group(P<0.05).Conclusion In microscopic single-level CSM surgery,zero-profile interbody fusion device reduces operative time and swallowing complication rates compared to titanium plates.
