摘要
杨栗山《伤寒瘟疫条辨》所载升降散,原为治温疫的首要方剂。张龙生主任师承国医大师薛伯寿,深得薛老的学术传承,重视气机升降,将升降散运用于头痛、颈动脉斑块、耳鸣等多种中医脑病治疗中,疗效良好。本文总结吾师运用升降散治疗中医脑病的临床经验,结合典型病例探讨其“升清降浊、调畅脑络”的独特治法。升降散通过蝉蜕、僵蚕升阳透邪,姜黄、大黄降浊逐瘀的配伍特点,符合脑病“清阳不升、浊阴不降”的核心病机。升降散通过调节气机、解毒化瘀,有效改善脑部微循环,抑制炎症反应,与现代医学的神经保护、抗氧化应激机制相契合。本文为经典方剂在脑病领域的拓展应用提供临床范例,体现“异病同治”的中医特色,为中医脑病防治提供新思路。
The Shengjiang powder,recorded in Yang Lishan's Differentiation and Treatment of Cold Damage and Pestilence was originally the primary prescription for treating pestilence.Director Zhang Longsheng studied under the National Master of Chinese Medicine Xue Boshou and deeply inherited Mr.Xue's academic teachings.He attaches great importance to the ascending and descending of qi mechanism.He has applied Shengjiang powder in the treatment of various encephalopathic diseases,such as headache,carotid artery plaques,tinnitus,and achieved good curative effects.This review summarizes my teacher's clinical experience in treating encephalopathic diseases in traditional Chinese medicine with Shengjiang powder.Discusses its unique treatment method of"lifting the clear and descending the turbid,and regulating the brain and collaterals"with typical cases,through the compatibility characteristics of Cicada Slough and Stiff Silkworm for raising yang and expelling pathogenic factors,and Turmeric Rhizome and Rhubarb for descending turbid substances and removing blood stasis,Shengjiang powder conforms to the core pathogenesis of encephalopathic diseases,which is"failure of the clear yang to ascend and failure of the turbid yin to descend."By regulating the qi mechanism,detoxifying and resolving blood stasis,Shengjiang powder can effectively improve the cerebral microcirculation,inhibit the inflammatory response,which is in line with the of neuroprotective and anti-oxidative stress mechanisms of modern medicine.This review provides a clinical example for expanding the application of classical formulae in the field of encephalopathic,reflecting the traditional Chinese medicine characteristic of"treating different diseases with the same method",and providing new ideas for the prevention and treatment of encephalopathic in traditional Chinese medicine.
