摘要
目的初步构建2型糖尿病气阴两虚兼血瘀证诊断量表,探索该证候诊断标准。方法通过文献分析初步筛选2型糖尿病气阴两虚兼血瘀证常见四诊信息以建立条目池,采用德尔菲法及层次分析法再次筛选、评估该病证诊断要素权重。结果经文献分析法共获得条目74个,根据频次法确定2型糖尿病气阴两虚兼血瘀证条目池,2轮德尔菲法收回有效问卷27份,专家积极系数分别为100%及80%,权威系数均>0.85,最终筛选出临床症状14种、舌象7种、脉象5种。层次分析结果显示一级指标比重临床症状>舌象>脉象;二级指标比重由高到低分别为临床症状:口渴喜饮、神疲乏力、气短懒言、咽干等;舌象:舌紫暗、舌红、舌少津苔干等;脉象:脉细、脉无力、脉涩等。结论基于文献分析、德尔菲法及层次分析法可有效筛选及评估2型糖尿病气阴两虚兼血瘀证诊断依据,为该病证诊断标准提供参考。
Objective To preliminarily establish the weight coefficients of diagnostic elements for type 2 diabetes mellitus(T2DM)with the syndrome of Qi-Yin deficiency and blood stasis,and to explore the diagnostic criteria for this syndrome.Methods Common diagnostic information for T2DM with Qi-Yin deficiency and blood stasis was initially screened through literature analysis to establish an item pool.The Delphi method and Analytic Hierarchy Process(AHP)were then employed to further refine and evaluate the weight coefficients of diagnostic elements.Results Seventy-four items were obtained through literature analysis.The item pool for T2DM with Qi-Yin deficiency and blood stasis was determined using the frequency method,including 14 clinical symptoms,7 tongue signs,and 5 pulse signs.Two rounds of the Delphi method recovered 27 valid questionnaires,with expert participation coefficients of 100%and 80%,and authority coefficients all>0.85.The AHP results showed that the priority of first-level indicators was clinical symptoms>tongue signs>pulse signs.Second-level indicators included:clinical symptoms(thirst with preference for drinking,fatigue,shortness of breath,sore throat,etc.);tongue signs(dark purple tongue,red tongue,dry tongue with little coating,etc.);and pulse signs(thready pulse,weak pulse,choppy pulse,etc.).Conclusion The combination of literature analysis,the Delphi method,and AHP can effectively screen and evaluate diagnostic criteria for T2DM with Qi-Yin deficiency and blood stasis,offering a reference for its diagnostic standard.
