AQP-4、6-Keto-PGF1α水平与高血压脑出血患者术后脑功能及预后的相关性

Correlation between AQP-4 and 6-Keto-PGF1αlevels with postoperative cerebral function and prognosis in patients with hypertensive intracerebral hemorrhage

摘要 目的在高血压脑出血患者中分析水通道蛋白4(aquaporin 4,AQP-4)、6-酮前列环素F1α(6-ketoprostacycline F1α,6-Keto-PGF1α)的表达水平与患者术后脑功能及预后的相关性。方法选取2021年9月—2023年9月手术治疗的99例高血压脑出血患者为观察对象,根据术后1个月的格拉斯哥预后评分将患者分为预后良好组和预后不良组,分析血清AQP-4、6-Keto-PGF1α的表达水平与患者术后脑功能(格拉斯哥-匹兹堡脑功能评分)的相关性;比较两组患者AQP-4、6-Keto-PGF1α水平,logistic回归分析AQP-4、6-Keto-PGF1α与高血压脑出血患者预后的相关性;建立受试者工作特征曲线评估AQP-4、6-Keto-PGF1α对高血压脑出血患者预后的预测价值。结果患者术后AQP-4的表达水平为(154.21±26.65)μg/L,6-Keto-PGF1α的表达水平为(14.86±3.42)pg/ml;AQP-4的表达水平与预后评分呈正相关,而6-Keto-PGF1α的表达水平与预后评分呈负相关(P<0.05);预后良好组AQP-4的表达水平低于预后不良组,6-Keto-PGF1α的表达水平高于预后不良组(P<0.05);高表达的AQP-4、低表达的6-Keto-PGF1α是高血压脑出血患者预后不良的危险因素(P<0.05);AQP-4、6-Keto-PGF1α预测高血压脑出血患者预后的曲线下面积分别为0.957和0.880(P<0.05)。结论AQP-4、6-Keto-PGF1α的表达水平与高血压脑出血患者术后脑功能密切相关,AQP-4、6-Keto-PGF1α是影响患者不良预后的相关危险因素,两者联合检测能够在临床上协助预测高血压脑出血预后不良的患者。 Objective To analyze the correlations of aquaporin 4(AQP-4)and 6-ketoprostacycline F1α(6-Keto-PGF1α)expression levels with postoperative cerebral function and prognosis in patients with hypertensive intracerebral hemorrhage.Methods Ninety-nine patients with hypertensive intracerebral hemorrhage who underwent surgical treatment at our hospital between September 2021 and September 2023 were enrolled.Based on Glasgow outcome scale(GOS)scores at 1-month postoperation,patients were categorized into favorable and poor prognosis groups.The correlation between AQP-4/6-Keto-PGF1αexpression levels and postoperative cerebral function(assessed by Glasgow-Pittsburgh cerebral performance scale)were analyzed,AQP-4 and 6-Keto-PGF1αlevels between groups were compared.Logistic regression was performed to examine their associations with prognosis,and receiver operating characteristic(ROC)curves were constructed to evaluate their predictive value for prognosis in hypertensive intracerebral hemorrhage patients.Results The postoperative expression of AQP-4 and 6-Keto-PGF1αwere(154.21±26.65)μg/L and(14.86±3.42)pg/ml in hypertensive intracerebral hemorrhage patients.AQP-4 showed significant positive correlation with prognosis scores,6-Keto-PGF1αdemonstrated negative correlation(P<0.05).Compared with the poor prognosis group,AQP-4 expression was significantly lower in the favorable prognosis group while 6-Keto-PGF1αexpression were markedly higher(P<0.05).Elevated AQP-4 and reduced 6-Keto-PGF1αwere independent risk factors for unfavorable outcomes(P<0.05).The area under the curve(AUC)of AQP-4 and 6-Keto-PGF1αin predicting the prognosis of hypertensive intracerebral hemorrhage patients were 0.957 and 0.880(P<0.05),respectively.Conclusions The expression levels of AQP-4 and 6-Keto-PGF1αwere significantly correlated with postoperative cerebral function in hypertensive intracerebral hemorrhage patients.Both biomarkers served as independent risk factors for poor prognosis.Their combined detection demonstrated superior predictive value for identifying high-risk patients in clinical practice.
