SIRI、eGFR、SCr水平与中老年冠心病患者冠状动脉狭窄程度的相关性研究

Study on correlation between SIRI,eGFR,SCr levels and the degree of coronary artery stenosis in middle-aged and elderly patients with coronary heart disease

摘要 目的探讨系统性炎症反应指数(SIRI)、肾小球滤过率(eGFR)、血清肌酐(SCr)水平与中老年冠状动脉粥样硬化性心脏病(CHD)患者冠状动脉狭窄程度的相关性。方法选取2021年6月—12月在新疆医科大学第五附属医院就诊的中老年CHD患者114例进行回顾性研究,根据Gensini评分的中位数分为轻度狭窄组(A组)和中重度狭窄组(B组)各57例。对比分析两组患者的临床及冠状动脉造影资料。结果B组SCr、SIRI水平高于A组,eGFR水平低于A组,差异有统计学意义(P<0.05)。多因素logistic回归分析表明,SIRI是CHD患者冠状动脉中重度狭窄的独立危险因素(OR=1.594,95%CI:1.044~2.435,P=0.031)。Gensini评分与SIRI、SCr呈正相关,与eGFR呈负相关(P<0.05)。eGFR、SCr和SIRI联合预测CHD患者冠状动脉为中重度狭窄的敏感度、特异度和AUC分别为0.404、0.877和0.664(95%CI:0.565~0.763)。结论SIRI、eGFR、SCr与冠状动脉狭窄程度均显著相关。SIRI是中老年CHD患者冠状动脉中重度狭窄的独立危险因素。SIRI联合eGFR、SCr水平对于预测冠状动脉狭窄程度具有较高的特异度。 Objective To investigate correlation between the systemic inflammation response index(SIRI),estimated glomerular filtration rate(eGFR),and serum creatinine(SCr)levels and the severity of coronary artery stenosis in middle-aged and elderly patients with coronary atherosclerotic heart disease(CHD).Methods A retrospective study was conducted on 114 elderly patients with CHD who were admitted to the Fifth Affiliated Hospital of Xinjiang Medical University from June to December 2021.According to the median Gensini score,they were divided into mild stenosis group(group A)and moderate to severe stenosis group(group B),with 57 patients in each group.And then,the clinical and coronary angiographic data of the two groups were compared and analyzed.Results The levels of SCr and SIRI in the group B were higher than those in the group A,while the eGFR level was lower,and the difference was statistically significant(P<0.05).Multivariate logistic regression analysis indicated that SIRI was an independent risk factor for moderate to severe coronary artery stenosis in patients with CHD(OR=1.594,95%CI:1.044–2.435,P=0.031).The Gensini score was positively correlated with SIRI and SCr,and negatively correlated with eGFR(P<0.05).The sensitivity,specificity and AUC of combined prediction of eGFR,SCr and SIRI for moderate-to-severe coronary artery stenosis in CHD patients were 0.404,0.877,and 0.664,respectively(95%CI:0.565–0.763).Conclusion SIRI,eGFR,and SCr are all significantly correlated with the degree of coronary artery stenosis.SIRI is an independent risk factor for moderate to severe coronary artery stenosis in middle-aged and elderly patients with CHD.The combination of SIRI,eGFR,and SCr levels has high specificity for predicting the degree of coronary artery stenosis.
