多重PCR诊断侵袭性真菌感染的价值分析

摘要 目的分析多重聚合酶链式反应(PCR)在侵袭性真菌感染(IFI)诊断中的应用价值,以提高早期检测率和准确性,为临床诊疗提供依据。方法选取50例IFI患者为研究对象,均进行多重PCR、传统真菌培养法检测。比较两种检测方法的侵袭性真菌阳性检出率、检测时间。结果多重PCR检出曲霉菌阳性15例、念珠菌阳性25例、新型隐球菌阳性5例,侵袭性真菌阳性检出率为90%(45/50);传统真菌培养法检出曲霉菌阳性9例、念珠菌阳性15例、新型隐球菌阳性3例,侵袭性真菌阳性检出率为54%(27/50)。多重PCR的侵袭性真菌阳性检出率高于传统真菌培养法(P<0.05)。多重PCR的检测时间为(6.5±1.2)h,传统真菌培养法的检测时间为(72.3±5.6)h,多重PCR的检测时间短于传统真菌培养法(P<0.05)。结论多重PCR检测技术在IFI诊断中具有更高的阳性检出率和准确性,并且能显著缩短检测时间,可作为早期诊断的重要工具,提高临床干预效率,改善患者预后。 Objective To analyze the application value of multiplex polymerase chain reaction(PCR)in the diagnosis of invasive fungal infection(IFI),to improve the early detection rate and accuracy,and to provide the basis for clinical diagnosis and treatment.Methods 50 patients with IFI were selected as study subjects,and underwent multiplex PCR and traditional fungal culture testing.The detection rate of positive invasive fungi and detection time of the two test methods were compared.Results Multiplex PCR detected 15 positive cases of Aspergillus,25 positive cases of Candida,and 5 positive cases of Cryptococcus neoformans,with a positive detection rate of invasive fungi of 90%(45/50);traditional fungal culture testing detected 9 positive cases of Aspergillus,15 positive cases of Candida,and 3 positive cases of Cryptococcus neoformans,with a positive detection rate of invasive fungi of 54%(27/50).The positive detection rate of invasive fungi by multiplex PCR was higher than that by traditional fungal culture testing(P<0.05).The detection time of multiplex PCR was(6.5±1.2)h,and that of traditional fungal culture testing was(72.3±5.6)h;the detection time of multiplex PCR was shorter than that of traditional fungal culture testing(P<0.05).Conclusion Multiplex PCR detection technology has higher positive detection rate and accuracy in the diagnosis of IFI,and significantly shorten the detection time.It can be used as an important tool for early diagnosis to improve the efficiency of clinical intervention and improve patient prognosis.
