摘要
目的:探讨中期因子(midkine,MK)、细胞角蛋白19(cytokeratin 19,CK19)在膀胱癌患者尿液中的表达情况,为膀胱癌的筛查和诊断提供参考依据。方法:采用病例对照研究方法,选取膀胱癌患者73例为病例组,同期健康体检者75例为对照组,采用ELISA法测定尿液中MK和CK19表达水平,分析尿液中MK和CK19表达水平与临床特征的关系及对膀胱癌的诊断价值。结果:膀胱癌患者尿液中MK和CK19表达水平均高于对照组(P<0.001);不同年龄、性别、BMI的膀胱癌患者尿液中MK和CK19表达水平差异无统计学意义(P>0.05),不同肿瘤数目和肿瘤大小患者尿液中MK和CK19表达水平差异无统计学意义(P>0.05),不同病理分级和肿瘤分型患者尿液中MK和CK19表达水平差异具有统计学意义(P<0.05),尿液MK诊断膀胱癌的ROC曲线下面积(AUC)为0.871(P<0.001),灵敏度为80.82%、特异度为82.67%;尿液CK19诊断膀胱癌的ROC曲线下面积为0.861(P<0.001),其灵敏度为82.19%、特异度为80.00%,二者联合诊断膀胱癌的曲线下面积为0.904(P<0.001),灵敏度为86.30%、特异度为80.00%。结论:膀胱癌患者尿液中MK和CK19表达水平高于正常人群,联合检测可以提高诊断的灵敏度,具有一定的临床价值。
Objective:To explore the expression of urinary midkine(MK)and cytokeratin 19(CK19)in urine of patients with bladder cancer and to provide reference basis for screening and diagnosis of bladder cancer.Methods:Using the method of case-control study,73 patients with bladder cancer were selected as the case group and 75 healthy people in the same period as the control group.The expression levels of MK and CK19 in urine were measured by enzyme-linked immunosorbent assay(ELISA).To analyze the relationship between the expression levels and clinical features of MK and CK19 in urine and its value in the diagnosis of bladder cancer.Results:As compared to the control groups,the expression levels of MK and CK19 were significantly increased in the bladder cancer group(P<0.001).There was no significant difference in the expression of MK and CK19 in urine of bladder cancer patients with different age,gender and body mass index(BMI)(P>0.05).There was no significant difference in the expression of MK and CK19 in urine of bladder cancer patients with different multiplicity and tumor size(P>0.05).There were significant differences in the expression levels of MK and CK19 in urine of patients with different pathological grades and tumor types(P<0.05).The diagnostic of bladder cancer was using receiver operating characteristic(ROC)curve analyses and the corresponding area under the curve(AUC)of urinary MK was 0.871(P<0.001)and the sensitivity and specificity of diagnosis were 80.82%and 82.67%,respectively.The AUC of urinary CK19 for the diagnosis of bladder cancer was 0.861(P<0.001)and the sensitivity and specificity of diagnosis were 82.19%and 80.00%,respectively.The AUC of the combination of two measures in the diagnosis of bladder cancer was 0.904(P<0.001)and the sensitivity and specificity of diagnosis were 86.30%and 80.00%,respectively.Conclusion:The expression levels of MK and CK19 in urine of patients with bladder cancer are higher than those of normal people.The detection of the combination of MK and CK19 could improve the sensitivity of diagnosis of bladder cancer and has a certain clinical value.
