摘要
目的:分析加长股骨近端防旋髓内钉(PFNA)内固定术治疗老年骨质疏松性股骨粗隆间骨折的效果。方法:选取2019年5月—2024年5月湖北省十堰市竹溪县人民医院收治的72例老年骨质疏松性股骨粗隆间骨折患者为研究对象,采用随机数字表法将其分为对照组和研究组,各36例。对照组采用传统PFNA内固定术治疗,研究组采用加长PFNA内固定术治疗。比较两组围手术期指标、疼痛程度、并发症发生情况、疗效。结果:两组手术时间、术中出血量对比,差异均无统计学意义(P>0.05);研究组术后住院时间、术后首次下床活动时间均短于对照组,差异均有统计学意义(P<0.05)。术后7 d,两组疼痛视觉模拟法评分均低于术前1 d,且研究组低于对照组,差异均有统计学意义(P<0.05)。研究组并发症发生率为5.56%,低于对照组的22.22%,差异有统计学意义(P<0.05)。研究组治疗总有效率为97.22%,高于对照组的83.33%,差异有统计学意义(P<0.05)。结论:加长PFNA内固定术治疗老年骨质疏松性股骨粗隆间骨折患者能有效缩短其恢复时间,缓解疼痛,降低并发症发生率,提高治疗效果。
Objective:To analysis the effect of extended proximal femoral nail anti-rotation(PFNA)internal fixation in the treatment of elderly patients with osteoporotic intertrochanteric fractures of femur.Methods:A total of 72 cases of elderly patients with osteoporotic intertrochanteric fractures of femur admitted to Zhuxi County People’s Hospital from May 2019 to May 2024 were selected as the study subjects,and were divided into a control group and a study group according to random number table method,with 36 cases in each group.The control group was treated with traditional PFNA internal fixation,while the study group was treated with extended PFNA internal fixation.The perioperative indexes,pain level,occurrence of complications and efficacy of the two groups were compared.Results:There were no significant differences in operation time and intraoperative blood loss between the two groups(P>0.05);the postoperative hospital stay and the first postoperative movement time from bed in the study group were shorter than those in the control group,the differences were statistically significant(P<0.05).Seven days after surgery,the Pain Visual Analogue Scale scores of both groups were lower than those of one day before surgery,and the study group was lower than the control group,the difference were statistically significant(P<0.05).The incidence rate of complications in the study group was 5.56%,which was lower than 22.22%in the control group,the difference was statistically significant(P<0.05).The total effective rate of the study group was 97.22%,which was higher than 83.33%of the control group,the difference was statistically significant(P<0.05).Conclusion:Extended PFNA internal fixation in the treatment of elderly patients with osteoporotic intertrochanteric fractures of femur can effectively shorten the recovery time,relieve pain,reduce the incidence of complications,and improve the treatment effect.
