摘要
目的:探究胫骨骨膜牵张术对糖尿病足溃疡患者炎症因子及足部相关指标的影响。方法:选取2020年5月—2022年12月山东省第二医科大学附属诸城市人民医院收治的80例糖尿病足溃疡患者为研究对象,按随机数字表法将其分为对照组及研究组,各40例。两组均进行常规治疗,在此基础上,对照组采用创面负压技术治疗,研究组采用创面负压技术联合胫骨骨膜牵张术治疗。两组均治疗4周。对比两组炎症因子水平、足部相关指标、疼痛程度及临床疗效。结果:治疗后,研究组超敏C反应蛋白、白细胞计数、白细胞介素-6、降钙素原水平均低于对照组,差异均有统计学意义(P<0.05)。治疗后,研究组的足温、趾端血氧、踝肱指数均高于对照组,差异均有统计学意义(P<0.05)。治疗后,研究组疼痛视觉模拟法评分低于对照组,差异有统计学意义(P<0.05)。研究组的治疗总有效率为95.00%,高于对照组的80.00%,差异有统计学意义(P<0.05)。结论:胫骨骨膜牵张术能减轻糖尿病足溃疡患者的炎症反应及疼痛程度,改善其足部相关指标具有良好疗效。
Objective:To explore the effects of tibial periosteal distraction on inflammatory factors and foot related indexes in patients with diabetic foot ulcers.Methods:A total of 80 patients with diabetic foot ulcers admitted to Zhucheng People’s Hospital Affiliated to Shandong Second Medical University from May 2020 to December 2022 were selected as the research objects and were divided into a control group and a study group according to the random number table method,with 40 cases in each group.Both groups received conventional treatment.On this basis,the control group was treated with wound negative pressure technology,and the study group was treated with wound negative pressure technology combined with tibial periosteal distraction.Both groups were treated for 4 weeks.The inflammatory factors level,foot related indexes,pain degree and clinical efficacy of the two groups were compared.Results:After treatment,the average levels of hypersensitive C-reactive protein,white blood cell count,interleukin-6,and procalcitonin in the study group were lower than those in the control group,the differences were statistically significant(P<0.05).After treatment,the foot temperature,toe tip blood oxygen and ankle-brachial index of the study group were higher than those of the control group,the differences were statistically significant(P<0.05).After treatment,the Pain Visual Analogue Scale score in the study group was lower than that in the control group,the difference was statistically significant(P<0.05).The total effective rate of treatment in the study group was 95.00%,which was higher than 80.00%in the control group,the difference was statistically significant(P<0.05).Conclusion:Tibial periosteal distraction can alleviate the inflammatory response and pain degree of patients with diabetic foot ulcers and improve the foot related indexes,has a good therapeutic effect.
