摘要
目的:探讨胆囊切除术致胆道损伤患者行腹腔镜下Roux-en-Y胆管空肠吻合术的临床治疗效果。方法:回顾性分析2022年1月—2023年12月商河县中医医院收治的60例胆囊切除术致胆道损伤患者的临床资料,按照治疗方法的不同将其分为对照组和研究组,各30例。对照组采用腹腔镜下胆道修补术联合T管引流术,研究组采用腹腔镜下Roux-en-Y胆管空肠吻合术。对比两组手术相关指标、实验室检查指标、术后近期及远期并发症发生情况。结果:两组术后首次排气时间、术后首次进食时间对比,差异均无统计学意义(P>0.05);研究组的术中出血量少于对照组、手术时间短于对照组,差异均有统计学意义(P<0.05)。术前及术后1周,两组总胆红素和尿淀粉酶水平对比,差异均无统计学意义(P>0.05)。研究组术后近期及远期并发症发生率分别为3.33%、23.33%,均低于对照组的26.67%、50.00%,差异均有统计学意义(P<0.05)。结论:腹腔镜下Roux-en-Y胆管空肠吻合术治疗胆囊切除术致胆道损伤患者的术中出血量较少、手术时间较短,术后近期及远期的并发症发生率较低,利于患者术后恢复。
Objective:To explore the clinical therapeutic effect of laparoscopic Roux-en-Y biliary jejunostomy in patients with biliary tract injury caused by cholecystectomy.Methods:A retrospective analysis was conducted on the clinical data of 60 patients with biliary tract injury caused by cholecystectomy admitted to Shanghe County Hospital of Traditional Chinese Medicine from January 2022 to December 2023.They were divided into a control group and a study group according to different treatment methods,with 30 cases in each group.The control group underwent laparoscopic biliary repair combined with T-tube drainage,while the study group underwent laparoscopic Roux-en-Y biliary jejunostomy.The surgery-related indicators,laboratory examination indicators,and the occurrence of short-term and long-term postoperative complications were compared between the two groups.Results:There was no statistically significant difference in the time of first exhaust after surgery and the time of first eating after surgery between the two groups(P>0.05).The intraoperative blood loss of the study group was less than that of the control group,and the operation time was shorter than that of the control group,the differences were statistically significant(P<0.05).Before the operation and one week after the operation,there was no statistically significant difference in the levels of total bilirubin and urinary amylase between the two groups(P>0.05).The incidences of short-term and long-term postoperative complications in the study group were 3.33%and 23.33%respectively,which were both lower than 26.67%and 50.00%in the control group,the differences were statistically significant(P<0.05).Conclusion:Laparoscopic Roux-en-Y biliary jejunostomy for patients with biliary tract injury caused by cholecystectomy has less intraoperative blood loss,shorter operation time,and lower rates of short-term and long-term postoperative complications,which is beneficial for postoperative recovery of patients.
