摘要
在经济全球化背景下,跨国人才流动愈加频繁,来华外国留学生对本地医疗健康服务的需求持续增长,其医疗保障获取与服务利用问题日益受到关注。为深入理解留学生的医疗保障需求及现实挑战,以医疗服务利用行为模型为理论基础,采用包含潜在的结构性可及、潜在的过程性可及、实现的客观可及和主观可及四个维度的医疗服务可及性分析框架,结合问卷调查与半结构式深度访谈,系统探讨在华留学生医疗服务可及性的现状、问题及其制度成因。研究发现,医疗健康服务信息获取的有效性与传递效率显著影响了留学生的医疗服务利用行为,进而影响其对医疗保障政策的认知水平、信任基础与实际利用率。语言支持不足、政策宣传滞后及参保渠道单一是制约信息获取的主要因素,限制了其潜在过程性与结构性可及,亦削弱了主观满意度。提升信息传递有效性与服务公平性,是改善医疗服务体验、推动社会保障体系国际化的重要路径。
In the context of economic globalization,transnational talent flows are becoming more frequent,and the demand of international students in China for local healthcare services continues to grow.Their access to medical security and service utilization are increasingly concerned.In order to gain a deeper understanding of the medical security demand and practical challenges of international students,this study takes the behavioral model of health services use as the theoretical basis,and adopts a healthcare service accessibility analysis framework that includes four dimensions:potential structural accessibility,potential procedural accessibility,objective accessibility,and subjective accessibility.It combines questionnaire surveys and semi-structured in-depth interviews to systematically explore the current situation,problems,and institutional causes of healthcare service accessibility for international students in China.Results show that the effectiveness and transmission efficiency of healthcare service information acquisition significantly affect the healthcare service utilization behavior of international students,which in turn affects their cognitive level,trust foundation,and actual utilization rate of medical security policies.Insufficient language support,lagging policy promotion,and a single channel for participating in insurance are the main factors are the main factors that restrict information acquisition.They limit its potential procedural and structural accessibility,and also weaken subjective satisfaction.Improving the effectiveness of information transmission and service fairness is an important path to enhance the healthcare service experience and promote the internationalization of the social security system.
