摘要
目的系统评价脑卒中患者衰弱的影响因素。方法计算机检索中英文数据库中关于脑卒中患者衰弱影响因素的研究,检索时限为建库至2024年6月1日。由2名研究者独立进行文献筛选、质量评价和数据提取,采用RevMan 5.3和Stata 17.0软件进行Meta分析。结果共纳入14篇文献。Meta分析结果显示:年龄增长[OR=1.07,95%CI(1.06,1.09)]、高龄[OR=3.83,95%CI(2.80,5.24)]、日常活动障碍[OR=2.59,95%CI(1.34,5.00)]、糖尿病[OR=3.99,95%CI(2.23,7.15)]、营养不良[OR=6.13,95%CI(2.93,12.82)]、美国国立卫生研究院卒中量表评分[OR=3.52,95%CI(2.18,5.68)]、睡眠障碍[OR=3.22,95%CI(1.66,6.25)]、抑郁[OR=4.79,95%CI(1.96,11.72)]、吞咽障碍[OR=2.31,95%CI(1.36,3.91)]、卒中次数[OR=4.39,95%CI(2.69,7.17)]和体育锻炼[OR=0.33,95%CI(0.21,0.50)]是脑卒中患者衰弱的影响因素。结论影响脑卒中患者衰弱的因素较多,医护人员应早期识别脑卒中衰弱高危人群,实施针对性的干预,以改善患者不良结局,提高脑卒中患者的生活质量。
Objective To evaluate systematically the influencing factors of frailty in stroke survivors.Methods Articles on the influencing factors of frailty in stroke survivors were searched in Chinese and English databases,with the time setting from the inception till June 1,2024.Literature screening,quality evaluation and data extraction were conducted independently by two researchers,and Meta-analysis was performed using RevMan 5.3 and Stata 17.0 software.Results A total of 14 articles were included.The results of Meta-analysis showed that age increasing[OR=1.07,95%CI(1.06,1.09)],old age[OR=3.83,95%CI(2.80,5.24)],disorder daily activites[OR=2.59,95%CI(1.34,5.00)],diabetes[OR=3.99,95%CI(2.23,7.15)],malnutrition[OR=6.13,95%CI(2.93,12.82)],high national institutes of health stroke scale score[OR=3.52,95%CI(2.18,5.68)],sleep disorder[OR=3.22,95%CI(1.66,6.25)],depression[OR=4.79,95%CI(1.96,11.72)],swallowing disorder[OR=2.31,95%CI(1.36,3.91)],stroke frequency[OR=4.39,95%CI(2.69,7.17)]and physical exercise[OR=0.33,95%CI(0.21,0.50)]were influencing factors of frailty in stroke survivors.Conclusion There are many factors affecting stroke patients′asthenia.Therefore,medical staff should identify the high-risk groups as early as possible and implement targeted interventions,avoiding the adverse outcomes and improving their life quality.
