特发性良性阵发性位置性眩晕成功复位后眩晕残障量表分析及残余症状疗效观察

Dizziness Handicap Inventory Assessment after Successful Reduction and Residual Symptoms Outcomes in Idiopathic Benign Paroxysmal Positional Vertigo

摘要目的探究特发性良性阵发性位置性眩晕(benign paroxysmal positional vertigo,BPPV)成功复位后眩晕残障量表(dizziness handicap inventory,DHI)评分及残余症状疗效。方法回顾性纳入东阳市人民医院耳鼻喉科2021年9月至2023年2月收治的117例特发性BPPV并成功复位患者为研究对象,根据患者是否存在残余症状分为残余组88例和无残余组29例,残余组根据简单随机分组方法分为观察组44例和对照组44例,对照组予以倍他司汀治疗,观察组在对照组基础上联合前庭康复训练。分析残余组与无残余组复位前后DHI评分,观察组和对照组残余症状持续时间、DHI评分、半年内复发情况。结果复位后,残余组和无残余组DHI评分均降低,无残余组DHI评分低于残余组,差异均有统计学意义(P<0.05);观察组残余症状持续时间低于对照组,差异有统计学意义(P<0.05);治疗后,观察组和对照组DHI评分均降低,观察组DHI评分低于对照组,差异有统计学意义(P<0.05);观察组半年内复发率与对照组比较,差异无统计学意义(P>0.05)。结论特发性BPPV成功复位后可能出现残余症状,经倍他司汀联合前庭康复训练治疗后症状得到有效改善。 Objective To report the Dizziness Handicap Inventory(DHI)scores and residual symptoms outcomes after successful reduction in patients with idiopathic benign paroxysmal positional vertigo(BPPV).Methods Data from patients with idiopathic BPPV and successfully treated with reduction(n=117)at the Department of Ootolaryngology of Dongyang People’s Hospital from September 2021 to February 2023 were retrospectively reviewed.Cases with residual symptoms(n=88)were divided into two groups based on treatments received,i.e.betahistine only(control group,n=44)or betahistine plus vestibular rehabilitation training(study group,n=44).DHI scores before and after reduction were compared between cases with residual symptoms and those without(n=29).For cases with residual symptoms,duration of residual symptoms,DHI score,and recurrence of BPPV within half a year were compared between the study and control groups.Results The DHI score was reduced in both cases with residual symptoms and those without after reduction therapies,and more so in the latter(P<0.05).In cases with residual symptoms,duration of residual symptoms in the study group was shorter than in the control group(P<0.05).After treatment,DHI score improved in both groups,more so in the study group(P<0.05),with no significant difference in recurrence between them within half a year(P>0.05).Conclusion Patients with idiopathic BPPV may have residual symptoms after successful reduction,which can be effectively improved by betahistine combined with vestibular rehabilitation training.
