摘要
目的探讨伦理氛围在道德型领导力和道德困境间的中介效应。方法采用便利抽样法,2023年8—9月,选取山东省5所三级甲等综合性医院606名临床护士为研究对象,采用伦理氛围量表、道德型领导力量表、道德困境量表进行调查,了解临床护士伦理氛围、道德型领导力和道德困境现状及相关性,构建并检验伦理氛围在道德型领导力和道德困境间的中介效应。结果本研究道德困境得分为1.00(0.23,2.10)分,道德型领导力得分为(6.06±1.18)分,伦理氛围得分为(4.23±0.57)分;Spearman相关性分析显示,伦理氛围与道德困境(r=-0.416,P<0.001)为负相关,伦理氛围与道德型领导力(r=0.717,P<0.001)为正相关,道德型领导力与道德困境(r=-0.245,P<0.001)为负相关。伦理氛围在道德型领导力和道德困境间起完全中介效应。结论临床护士道德困境得分较低,道德型领导力和伦理氛围得分处于较高水平,道德型领导力可以通过伦理氛围降低临床护士道德困境。建议医疗保健体系加强对护理管理者道德素养的培养,通过提升护理管理团队的管理水平,营造科室良好的伦理氛围,降低道德困境对临床护士造成的心理压力,从而进一步稳定护理队伍。
Objective To explore the mediating effect of ethical climate between ethical leadership and moral distress.Methods By convenient sampling,606 clinical nurses from 5 tertiary grade-A general hospitals in Shandong Province were selected as the research objects from August to September 2023,and The Ethical Climate Scale,Moral Leadership Scale and Moral Distress Scale were used to understand the current status and correlation of clinical nurses′ethical climate,moral leadership,and moral distress.Mediating effect of ethical climate between moral leadership and moral distress was explored.Results In this study,the score of moral distress,ethi⁃cal leadership and ethical climate was 1.00(0.23,2.1),6.06±1.18,and 4.23±0.57,respectively;spearman correlation analysis showed that ethical climate was negatively correlated with moral distress(r=-0.416,P<0.001),and positively correlated with ethical leadership(r=0.717,P<0.001);ethical leadership was negatively correlated with moral distress(r=-0.245,P<0.001).Ethical climate had a com⁃plete mediating effect between moral leadership and moral distress.Conclusion The score of moral dilemma of clinical nurses is lower,but that of moral leadership and ethical climate are higher.Moral leadership could reduce moral dilemma of clinical nurses through ethical atmosphere.Ethical literacy and management competency of nursing managers should be enhanced to create a favorable ethical climate in the department and reduce psychological pressure caused by moral distress among clinical nurses,thus to stabilize nursing team.
