摘要
目的探讨血清C肽、胰岛素(insulin,INS)及尿微量白蛋白(microalbuminuria,mAlb)单独及联合检测在糖尿病肾病(diabetic nephropathy,DN)早期诊断中的灵敏度和特异度,为临床早期诊断提供参考。方法回顾性选取2023年2月—2024年11月于陆军第七十三集团军医院就诊的44例DN患者(研究组)及56例单纯2型糖尿病患者(对照组)的临床资料。两组均检测血清C肽、INS及mAlb水平,比较两组指标水平,并评估上述指标单独及联合检测对DN的诊断效能。结果研究组血清C肽、INS及mAIb均比对照组更高,差异均有统计学意义(P均<0.05)。联合检测的灵敏度、特异度、准确度、阳性预测值、阴性预测值均高于血清C肽、INS、mAlb单一检测,差异均有统计学意义(P均<0.05)。结论血清C肽、INS及mAlb水平联合检测可有效提高DN的诊断效能及一致性,为DN的诊断提供有力支持。
Objective To explore the sensitivity and specificity of the individual and combined detection of serum C-peptide,insulin(INS),and microalbuminuria(mAlb)in the early diagnosis of diabetic nephropathy(DN),and to pro⁃vide a reference for the early clinical diagnosis.Methods The clinical data of 44 patients with DN(study group)and 56 patients with simple type 2 diabetes mellitus(control group)who were treated at the PLA 73rd Group Army Hospi⁃tal from February 2023 to November 2024 were retrospectively selected.The levels of serum C-peptide,INS and mAlb were detected in both groups.The levels of the indicators in the two groups were compared,and the diagnostic efficacy of the above indicators alone and in combination for DN was evaluated.Results The levels of serum C-peptide,INS and mAIb in the study group were all higher than those in the control group,and the differences were sta⁃tistically significant(all P<0.05).The sensitivity,specificity,accuracy,positive predictive value and negative predic⁃tive value of the combined detection were all higher than those of the single detection of serum C-peptide,INS and mAlb,and the differences were statistically significant(all P<0.05).Conclusion The combined detection of serum C-peptide,INS and mAlb levels can effectively improve the diagnostic efficacy and consistency of DN,providing strong support for the diagnosis of DN.
