温控射频联合功能性磁刺激对盆底功能障碍患者盆底肌纤维肌力及盆底功能的影响

Effect of temperature-controlled radiofrequency combined with functional magnetic stimulation on the muscle strength and pelvic floor function of patients with pelvic floor dysfunction

摘要 目的:回顾性分析温控射频联合功能性磁刺激在盆底功能障碍(Pelvic floor dysfunction,PFD)中的应用效果。方法:收集郸城县妇幼保健院2023年5月-2024年5月122例PFD患者临床资料,根据治疗方法不同分为对照组(n=61)、观察组(n=61),对照组给予功能性磁刺激治疗,观察组在此基础上联合温控射频疗法。比较两组盆底肌纤维肌力、盆底功能、盆底肌表面肌电指标、血清松弛素含量。结果:干预12 w后,两组Ⅰ、Ⅱ类肌纤维肌力均有所改善,观察组肌力达Ⅲ级以上比例高于对照组(P<0.05);治疗12 w后观察组盆底功能障碍问卷(Pelvic Floor Disorders Impact Questionnaire-20,PFDI-20)评分显著低于对照组(P<0.05);治疗12 w后,观察组血清松弛素含量低于对照组,盆底肌表面肌电指标高于对照组(P<0.05)。结论:温控射频联合功能性磁刺激疗法可促进PFD患者盆底功能恢复,提高盆底肌纤维肌力,降低血清松弛素水平,单纯功能性磁刺激效果不明显。 Objective:To retrospectively analyze the application effect of temperature-controlled radiofrequency combined with functional magnetic stimulation in the treatment of pelvic floor dysfunction(PFD).Methods:Clinical data of 122 patients with PFD from May 2023 to May 2024 in Dancheng County Maternal and Child Health Hospital were collected and divided into a control group(n=61)and an observation group(n=61)according to different treatment methods.The control group was treated with functional magnetic stimulation,while the observation group was treated with temperature-controlled radiofrequency therapy in addition to functional magnetic stimulation.The muscle strength of pelvic floor muscles,pelvic floor function,surface electromyography indicators of pelvic floor muscles,and serum relaxin content were compared between the two groups.Results:After 12 weeks of intervention,the muscle strength of both type I and type II muscle fibers in both groups were improved,and the proportion of patients with muscle strength above grade III in the observation group was higher than that in the control group(P<0.05).After 12 weeks of treatment,the PFDI-20 score in the observation group was significantly lower than that in the control group(P<0.05).After 12 weeks of treatment,the serum relaxin content in the observation group was lower than that in the control group,and the surface electromyography index of pelvic floor muscles was higher than that in the control group(P<0.05).Conclusion:Thermally controlled radiofrequency combined with functional magnetic stimulation therapy can promote the recovery of pelvic floor function in patients with PFD,improve the muscle strength of pelvic floor muscles,and reduce the level of serum relaxin.The effect of functional magnetic stimulation alone is not significant.
