摘要
心肾综合征虽责之心肾不交,上下同病,但却与脾胃关系密切。一方面,脾胃气虚,生化乏源,气血不足,则心肾无资,水火变通无源,致心肾不交;另一方面,脾失健运,水湿、痰饮、瘀血、浊毒等有形实邪内生,使清气不升,浊阴不降,中枢转运失常,水火不得升降,亦可致心肾不交。治疗上应调理中焦脾胃,具体又可分为补脾益气、活血化瘀,健脾助运、补气升提,益气养血、固表防变,利水除湿、温阳化饮,和胃泻浊、调畅气机等治法。临床上,应结合患者具体情况,多法并用,调脾胃以资化源、复气机、顺升降、交心肾,有助于提高治疗心肾综合征的临床疗效。
Although it is due to the discord between the Heart and Kidney,cardiorenal syndrome is closely related to the Spleen and Stomach.On the one hand,if the Spleen and Stomach Qi are deficient,Qi and blood production are insufficient.The Heart and Kidney deficiency,the Water and Fire will be incompatible,resulting in the Heart and Kidney not communicating well;On the other hand,the when Spleen loses its health,Water and Dampness,Phlegm,blood stasis,Turbidity and other tangible Evil Factors are endogenous,so that the clear Qi does not rise,the Turbid Yin does not fall,the central transport is abnormal,and the Water and Fire cannot rise and fall,which can also cause the Heart and Kidney not communicating well.The treatment emphasizes the conditioning of the Spleen and Stomach,specifically,nourishing the Spleen and nourishing Qi,invigorating blood circulation and dissolving blood stasis,strengthening the Spleen and helping transportation,nourishing Qi and raising Qi,nourishing Qi and blood,fixing the surface and preventing degeneration,diluting Water and dehumidifying,warming Yang to relieve Phlegm,harmonizing Stomach and lower the turbidity,as well as regulating the Qi activity of the whole body.Clinically,it is necessary to combine multiple methods accordingly,to adjust the Spleen and Stomach,to better Qi activity,to regulate Qi going up and down,and eventually to help improve the clinical efficacy of cardiorenal syndrome treatment.
