摘要
目的采用自制健瘘操动画视频指导新建内瘘患者术后锻炼,为其提供系统、简明的锻炼方法,以期通过生动形象的教育方式提高患者的锻炼依从性,从而促进内瘘成熟。方法选取2022年1月至2024年1月行自体动静脉内瘘成形术维持性血透患者60例,随机分为观察组(n=30)与对照组(n=30)。观察组采用自制健瘘操动画视频进行内瘘术后教育,对照组予常规内瘘术后教育。两组均予自制督导表记录内瘘锻炼、自我监测情况。术后干预4周、随访12周。比较两组内瘘成熟率、内瘘血管情况,包括头静脉内径、桡动脉内径、肱动脉流量和内瘘锻炼及自我监测依从性。比较两组内瘘成熟不良率。结果手术4周后,观察组内瘘成熟率53.33%,高于对照组33.33%,观察组血管内径、血流量均优于对照组,观察组患者内瘘锻炼与自我监测依从性均高于对照组,差异均有统计学意义(P<0.05)。手术12周后,观察组内瘘成熟不良率0.00%,低于对照组20.00%,差异有统计学意义(P<0.05)。结论健瘘操动画视频可提高患者内瘘锻炼依从性,进而在扩大内瘘血管内径,增加血流量,提高成熟率等方面比常规术后教育有更好的促进作用,值得临床推广使用。
Objective To provide systematic and concise exercise methods for patients with newly established internal fistula by using self-made animation video of fistula exercise,so as to improve the exercise compliance of patients and promote the maturity of internal fistula.Methods From January 2022 to January 2024,60 patients who underwent autogenous arteriovenous fistula(end-to-side anastomosis of left forearm radial artery-cephalic venous fistula)in our hospital and received maintenance hemodialysis in our hospital were selected as the research objects.They were divided into the experimental group(n=30)and the control group(n=30)by random number table method.The experimental group used the self-made animation video of healthy fistula exercise for postoperative education of internal fistula,while the control group received routine postoperative education of internal fistula.The self-made supervision table was used to record the fistula exercise and self-monitoring in both groups.Postoperative intervention lasted for 4 weeks and follow-up for 12 weeks.Four weeks after surgery,the maturity rate of internal fistula,internal fistula vessels,including cephalic vein diameter,radial artery diameter,brachial artery flow,internal fistula exercise and self-monitoring compliance were compared between the two groups.After 12 weeks of operation,the rate of fistula immaturity was compared between the two groups.Results Four weeks after the operation,the fistula maturation rate of the experimental group was 53.33%,which was higher than 33.33%of the control group.The results of blood vessel diameter and flow showed by B-ultrasound in the experimental group were better than those in the control group.The compliance of internal fistula exercise and self-monitoring of patients in the experimental group were higher than those in the control group,and the differences all were statistically significant(P<0.05).After 12 weeks of operation,the rate of fistula maturation in the experimental group was 0.00%,which was lower than 20.00%in the control group,and the difference was statistically significant(P<0.05).Conclusion The animation video of fistula health exercise can improve the compliance of patients with internal fistula exercise,and has a better promoting effect than conventional postoperative education in expanding the inner diameter of internal fistula vessels,increasing blood flow and improving the maturity rate,which is easy to be used in clinical practice.
