摘要
目的:探讨使用心率变异性指标(HRV)辅助诊断阻塞性睡眠呼吸暂停低通气综合征(OSAS)患者心血管疾病的可行性。方法:选取2022年1月至2024年4月于陕西省第二人民医院耳鼻喉科以打鼾为主诉接受治疗的120例患者作为研究对象,同步进行动态心电图检查,根据低通气指数(AHI)对OSAS进行分组,分别为单纯鼾症组(n=34)、轻度OSAS组(n=30)、中度OSAS组(n=33)及重度OSAS组(n=23),比较4组患者HRV数据及心律失常数据。结果:单纯鼾症组R-R间期平均标准差(SDNN)、每5 min R-R间期平均标准差(SDANN)明显高于轻度组、中度组及重度组,而低频(LF)指标低于轻度组、中度组及重度组;重度组患者SDNN、SDANN、高频(HF)明显低于轻度组及中度组,而LF高于轻度组和中度组;重度组患者心律失常发生率明显高于单纯鼾症组、轻度组及中度组(P<0.05)。结论:心率变异性指标对于辅助诊断OSAS患者心血管疾病的可行性高,临床可根据情况选择使用。
Objective:To explore the feasibility of heart rate variability(HRV)indicators in assisting in the diagnosis of cardiovascular disease among patients with obstructive sleep apnea-hypopnea syndrome(OSAS).Methods:A total of 120 patients presented with snoring and treated in the Department of Otolaryngology,Second People Hospital of Shaanxi Province from January 2022 to April 2024 were collected.They underwent dynamic electrocardiogram examination at the same time and were classified into simple snoring group(n=34),mild OSAS group(n=30),moderate OSAS group(n=33)and severe OSAS group(n=23)according to the degree of OSAS calculated by the apnea hypopnea index(AHI).HRV data and arrhythmia data were compared among the four groups.Results:The standard deviation of NN intervals(SDNN)and the standard deviation of the average normal-to-normal RR intervals(SDANN)every 5 min in the simple snoring group were significantly higher than the mild,moderate and severe OSAS groups,while the low frequency(LF)was significantly shorter(P<0.05).SDNN,SDANN and high frequency(HF)were significantly lower in the severe OSAS group than the mild and moderate OSAS groups,while LF was significantly higher(P<0.05).The incidence of arrhythmia was significantly higher in the severe OSAS group than the simple snoring group and mild and moderate OSAS groups(P<0.05).Conclusion:HRV indicators are highly feasible for assisting in the diagnosis of cardiovascular disease in OSAS patients,and can be used clinically according to the situation.
