摘要
目的:探讨经外周静脉置入中心静脉导管(PICC)相关性皮肤损伤(CASI)病人生活质量的发展轨迹,并分析其影响因素。方法:选取福建省某三级甲等肿瘤专科医院的162例发生PICC导管相关性皮肤损伤病人作为研究对象,采用一般资料调查表、接触性皮炎评估表、皮肤病生活质量量表分别于皮肤损伤发生时、发生后1个月以及发生后2个月进行追踪调查,利用潜变量增长混合模型及无序多分类Logistic回归分析影响因素。结果:皮肤损伤发生时、发生后1个月以及发生后2个月时病人的生活质量得分分别为(10.08±4.89)分、(12.65±4.33)分、(15.09±6.19)分;病人生活质量发展轨迹可分为中度影响-稳定组(50.6%)、高度影响-改善组(7.4%)、中度影响-加重组(42.0%);回归分析显示,年龄、患基础疾病种类、延迟维护次数和接触性皮炎是病人生活质量的影响因素(P<0.05)。结论:PICC导管相关性皮肤损伤病人生活质量随病情变化发展轨迹存在异质性,医护人员可根据影响因素制订针对性护理措施,提高病人的生活质量。
Objective:To explore the developmental trajectory of quality of life(QOL)in patients with peripherally inserted central catheter(PICC)-associated skin impairment(CASI)and to analyze its influencing factors.Methods:A total of 162 patients with PICC-related CASI from a tertiary tumor hospital in Fujian province were enrolled.General information questionnaires,the Contact Dermatitis Assessment Scale,and the Dermatology Life Quality Index(DLQI)were used for longitudinal assessments at three time points:at the onset of CASI(T1),one month post-onset(T2),and two months post-onset(T3).A latent class growth mixture model(GMM)and multinomial logistic regression were employed to analyze influencing factors.Results:The QOL scores of CASI patients at T1,T2,and T3 were(10.08±4.89),(12.65±4.33),and(15.09±6.19),respectively.The developmental trajectories of QOL were categorized into three groups:moderate impact-stable(50.6%),high impact-improving(7.4%),and moderate impact-worsening(42.0%).Regression analysis indicated that age,number of underlying diseases,delayed catheter maintenance,and contact dermatitis were significant influencing factors(P<0.05).Conclusions:The QOL of CASI patients exhibits heterogeneous developmental trajectories influenced by disease progression.Healthcare providers should develop targeted nursing interventions based on these factors to improve patients′QOL.
