摘要
目的:探究应用磁共振多翻转角T1 mapping结合血清细胞周期素A2(CCNA2)、高尔基体糖蛋白-73(GP73)诊断肝癌的影像学特征及诊断效能。方法:选择2023年1月至2024年1月收治的65例疑似肝癌患者作为研究对象,以病理结果为金标准,确定48例为肝癌患者,其余17例为肝病变,均进行磁共振多翻转角T1 mapping和血清CCNA2、GP73检测,分析磁共振多翻转角T1 mapping的诊断准确率、灵敏度、特异度。结果:肝癌患者CCNA2、GP73水平高于肝病变患者,差异有统计学意义(P<0.05)。从高分化到低分化肝癌组,T1post呈递增趋势,ΔT1%呈递减趋势。应用磁共振多翻转角T1 mapping结合血清CCNA2、GP73诊断肝癌较磁共振多翻转角T1 mapping单独诊断阳性检出率高,灵敏度、特异度、准确度高。结论:磁共振多翻转角T1 mapping结合血清CCNA2、GP73对肝癌具有较高的诊断价值,临床诊断时可优先考虑。
Objective:To investigate the imaging characteristics and diagnostic efficacy of applying magnetic resonance multi-flip angle T1 mapping combined with serum CCNA2 and GP73 to diagnose hepatocellular carcinoma.Methods:Sixty-five patients with suspected hepatocellular carcinoma admitted to our hospital from January 2023 to January 2024 were selected as the study subjects,and 48 cases were identified as hepatocellular carcinoma patients,and the remaining 17 cases were hepatopathology using the pathological results as the gold standard,all of which were subjected to magnetic resonance multi-flip angle T1 mapping,serum CCNA2 and GP73 testing,and were analyzed for the diagnostic accuracy of magnetic resonance multi-flip angle T1 mapping,sensitivity and specificity.Results:The levels of CCNA2 and GP73 in patients with hepatocellular carcinoma were higher than those in patients with liver lesions,and there was a difference(P<0.05).From highly differentiated to poorly differentiated hepatocellular carcinoma group,T1post showed an increasing trend andΔT1%showed a decreasing trend.In the present study,using the pathologic results as the"gold standard",we found that the positive detection rate of magnetic resonance multi-rotation angle T1 mapping combined with serum CCNA2 and GP73 was high in the present study.The sensitivity,specificity and accuracy of magnetic resonance multi-rotation angle T1 mapping combined with serum CCNA2 and GP73 were higher than those of magnetic resonance multi-rotation angle T1 mapping alone.Conclusion:Magnetic resonance multiple flip angle T1 mapping combined with serum CCNA2 and GP73 has high diagnostic value and can be prioritized in clinical diagnosis.
