摘要
目的:探讨逍遥散加减联合鳖甲煎丸对肝癌经肝动脉导管化疗栓塞(TACE)术后综合征的治疗效果及对患者免疫功能的影响。方法:纳入2020年1月至2024年6月收治的肝癌TACE术后综合征患者120例,根据治疗方案分为3组,每组40例,汤药组患者以逍遥散加减治疗,成药组患者以鳖甲煎丸治疗,联药组患者以逍遥散加减联合鳖甲煎丸治疗,评价患者治疗效果、TACE术后综合征持续时间、免疫功能。结果:联药组患者治疗效果优于成药组、汤药组(P<0.05),成药组与汤药组患者疗效差异无统计学意义(P>0.05),联药组患者TACE术后综合征持续时间短于汤药组、成药组(P<0.05),汤药组、成药组患者TACE术后综合征持续时间差异无统计学意义(P>0.05),治疗前3组患者免疫功能指标差异无统计学意义(P>0.05),治疗后联药组患者CD4^(+)、IgA、IgG、IgM水平高于汤药组和成药组(P<0.05),CD8^(+)水平低于汤药组和成药组(P<0.05),成药组和汤药组患者免疫功能指标差异无统计学意义(P>0.05)。结论:肝癌TACE术后综合征患者以逍遥散加减联合鳖甲煎丸治疗,疗效优于单一用药,可有效缩短患者TACE术后综合征持续时间,改善免疫功能。
Objective:To explore the therapeutic effect of modified Xiaoyao San combined with Biejiajian Wan on post-TACE syndrome of liver cancer and its impact on the immune function of patients.Methods:The subjects(n=120)of this study were all patients with post-TACE syndrome of liver cancer admitted to our hospital from January 2020 to June 2024.They were divided into three groups according to different treatment regimens.Decoction Group(n=40)was treated with Xiaoyao San,Patent Medicine Group(n=40)was treated with Biejiajian Wan,and Combined Medication Group(n=40)was treated with modified Xiaoyao San combined with Biejiajian Wan.The treatment effect,duration of post-TACE syndrome,and levels of immune function indicators of the patients were evaluated.Results:Compared with the patent and decoction groups,the combined group achieved a significantly better overall response(P<0.05),whereas no difference was observed between the patent and decoction groups(P=0.42).The median duration of post-TACE syndrome was also shorter in the combined group than in the patent or decoction group(P<0.05),with no significant difference between the latter two(P=0.38).Baseline immune indices did not differ among the three groups(P>0.05).After treatment,the combined group exhibited higher CD4^(+)counts and serum IgA,IgG and IgM levels,together with lower CD8^(+)counts,than the patent or decoction group(P>0.05);immune indices remained comparable between the patent and decoction groups(P>0.05).Conclusion:For patients with post-TACE syndrome of liver cancer,the treatment with modified Xiaoyao San combined with Biejiajian Wan is superior to single drug use,which can effectively shorten the duration of post-TACE syndrome and improve immune function.This combined regimen has high reference value in clinical practice.
