摘要
目的:通过梳理DRG病种成本核算研究成果,了解研究主题和进展,为进一步开展病种成本核算研究提供参考。方法:以中国知网(CNKI)、万方数据库检索出的文献为数据来源,利用CiteSpace对纳入文献进行关键词共现、聚类和突变分析。结果:研究总体呈上升趋势,但发展相对缓慢,2015年后发文量快速提升。从研究热点上看,现有研究注重对单病种的住院费用进行分析,对核算体系构建和临床路径的协同效应进行研究。从研究进程上看,现有研究经历了初步探索期、试点推广期和规范发展期,体现出显著的政策驱动特征;医疗机构在实施核算中面临病种、方法选择、数据质量和信息系统构建等多方面挑战。结论:未来,构建科学的核算体系,基于核算结果进行精细化运营管理和绩效激励是主要研究方向。
Objective:To review research achievements on DRG disease cost accounting,understand the research themes and progress,and provide references for further studies on disease cost accounting.Methods:Literature retrieved from CNKI and Wanfang databases was used as the data source,and CiteSpace was employed to conduct keyword co-occurrence,clustering,and burst analysis on the included studies.Results:The overall research showed an upward trend,but the development was relatively slow until a rapid increase in publications after 2015.In terms of research hotspots,existing studies focus on the analysis of hospitalization costs of single diseases and the study of accounting system construction and the synergistic effect of clinical pathways.In terms of research process,studies have gone through preliminary exploration,pilot promotion,and standardized development stages,showing significant policy-driven characteristics.Medical institutions face multiple challenges in implementing accounting,including disease selection,methodological choices,data quality,and information system construction.Conclusion:In the future,constructing a scientific accounting system and conducting refined operational management and performance incentives based on accounting results will be the main research directions.
