摘要
目的:评估经尿道钬激光前列腺剜除术(HoLEP)中最大限度保留功能尿道的改良术式的临床效果。方法:选取2024年1月—12月山东第二医科大学附属医院收治的216例良性前列腺增生(BPH)患者的临床资料,其中采用保留功能尿道改良术式(A组)104例,传统术式(B组)112例。评估围手术期指标、有效性指标[术后第1个月、3个月、6个月的国际前列腺症状评分(IPSS)、生活质量(QoL)和残余尿量(PVR)]、安全性指标[拔除导尿管后即刻及术后第1周、2周、4周压力性尿失禁(SUI)发生率]。结果:两组围手术期指标比较,差异无统计学意义(P>0.05)。A组术后第6个月IPSS评分显著优于B组,差异有统计学意义(P<0.05)。在安全性方面,A组在拔管即刻及术后第1周、2周、4周的压力性尿失禁发生率均显著低于B组,差异有统计学意义(P<0.05)。结论:最大限度保留功能尿道的改良HoLEP术式在保证手术疗效的同时,可显著降低术后尿失禁的发生率,具有较好的临床应用价值。
Objective:To evaluate the clinical outcomes of modified Holmium Laser Enucleation of the Prostate(HoLEP)with maximum preservation of functional urethra.Method:A retrospective analysis was conducted on 216 patients with benign prostatic hyperplasia(BPH)treated in the hospital from January to December 2024,including 104 cases using modified HoLEP with functional urethral preservation(Group A)and 112 cases using traditional HoLEP(Group B).Assessment indicators included perioperative parameters,effectiveness indicators(IPSS score,QoL score,and PVR 1,3,and 6 months postoperatively);and safety indicators[incidence of stress urinary incontinence(SUI)immediately after catheter removal and 1,2,and 4 weeks after operation].Result:All surgeries were successfully completed,and there were no significant differences in perioperative parameters between the two groups(P>0.05).Group A showed significantly better IPSS scores than Group B at 6 months postoperatively,and the difference was statistically significant(P<0.05).Regarding safety,Group A demonstrated significantly lower incidence of stress urinary incontinence both immediately after catheter removal and 1,2,and 4 weeks postoperatively compared to Group B,and the differences were statistically significant(P<0.05).Conclusion:The modified HoLEP technique with maximum preservation of functional urethra maintains surgical efficacy while significantly reduces the incidence of postoperative urinary incontinence,demonstrating promising clinical value.
