摘要
目的 探讨新生儿危重病例评分(NCIS)、新生儿急性生理学评分-Ⅱ(SNAP-Ⅱ)和新生儿急性生理学评分围生期补充-Ⅱ(SNAPPE-Ⅱ)对新生儿呼吸窘迫综合征(NRDS)患儿死亡的预测价值。方法 随访12个月,将192例NRDS患儿分为死亡组29例和存活组163例。比较两组临床一般资料,绘制ROC曲线评估NCIS、SNAP-Ⅱ及SNAPPE-Ⅱ单独或联合应用对NRDS患儿死亡的预测价值。结果 两组新生儿出生体重比较有统计学差异(P<0.05),两组性别构成比、胎龄、分娩方式比较无统计学差异(P>0.05)。NCIS、SNAP-Ⅱ及SNAPPE-Ⅱ单独或联合应用对NRDS患儿死亡均具有预测价值(P<0.01),NCIS的AUC>SNAPPE-Ⅱ的AUC>SNAP-Ⅱ的AUC(P<0.05),NCIS联合SNAPPE-Ⅱ的AUC>NCIS联合SNAP-Ⅱ的AUC>三种联合的AUC>SNAP-Ⅱ联合SNAPPE-Ⅱ的AUC(P<0.05)。结论 三种评分单独或联合应用均可以预测NRDS患儿死亡,NCIS单独或联合SNAPPE-Ⅱ对NRDS患儿死亡的预测价值较高。
Objective To explore the predictive value of the combined application of neonatal critical illness score(NCIS),score for neonatal acute physiology-Ⅱ(SNAP-Ⅱ)and score for neonatal acute physiology perinatal extension-Ⅱ(SNAPPE-Ⅱ)for the death of children with neonatal respiratory distress syndrome(NRDS).Methods During the 12-month follow-up,192 children with NRDS were divided into death group of 29 cases and survival group of 163 cases.The general clinical data of the two groups were compared and the ROC curve was drawn to evaluate the predictive value of NCIS,SNAP-Ⅱ and SNAPPE-Ⅱalone and in combination for the death of children with NRDS.Results There was significant difference in the birth weight of neonates between the two groups(P<0.05),while there was no significant differences in the gender,gestational age and delivery mode between the two groups(P>0.05).The NCIS,SNAP-Ⅱand SNAPPE-Ⅱalone or in combination had predictive value for the death of children with NRDS(P<0.01).The AUC of NCIS was greater than that of the SNAPPE-Ⅱ,which in turn was greater than that of SNAP-Ⅱ(P<0.05).The AUC of combination of NCIS and SNAPPE-Ⅱwas greater than that of combination of NCIS and SNAP-Ⅱ,which was greater than that of combination of all three scores,and which was in turn greater than that of combination of SNAP-Ⅱ and SNAPPE-Ⅱ(P<0.05).Conclusion The application of three scores alone or in combination can predict death of children with NRDS,NCIS alone or in combination with SNAPPE-Ⅱ have the higher predictive value for death of children with NRDS.
