炎症性肠病患者肠道菌群分布特征及其与血浆外泌体微小RNA-214-3p和微小RNA-184的关系

Distribution characteristics of intestinal microflora in patients with inflammatory bowel disease and their relationship with miR-214-3p and miR-184 in plasma exosomes

摘要 目的 探讨炎症性肠病(IBD)患者肠道菌群分布特征及其与血浆外泌体微小RNA-214-3p(miR-214-3p)和miR-184的关系。方法 检测98例IBD患者(观察组)和同期100例健康体检者(对照组)的肠道菌群,采用实时荧光定量PCR检测血浆外泌体miR-214-3p和miR-184表达。比较两组肠道菌群数量、肠道菌群阳性率、血浆外泌体miR-214-3p和miR-184表达,采用Pearson相关分析观察组血浆外泌体miR-214-3p和miR-184表达与菌群数量的关系。采用ROC曲线评估血浆外泌体miR-214-3p和miR-184对IBD的诊断价值。结果 观察组双歧杆菌、拟杆菌、肠球菌、酵母菌和肠杆菌数量均高于对照组,真杆菌数量低于对照组(P<0.05)。观察组双歧杆菌和酵母菌菌群阳性率高于对照组,真杆菌菌群阳性率低于对照组(P<0.05)。观察组血浆外泌体miR-214-3p表达及血清IL-6、TNF-α、hsCRP水平高于对照组,血浆外泌体miR-184表达低于对照组(P<0.05)。血浆外泌体miR-214-3p表达与双歧杆菌、拟杆菌、肠球菌、酵母菌、肠杆菌数量及血清IL-6、TNF-α、hsCRP水平呈正相关,与真杆菌数量呈负相关(P<0.05);血浆外泌体miR-184表达与真杆菌数量呈正相关,与双歧杆菌、拟杆菌、肠球菌、酵母菌、肠杆菌数量及血清IL-6、TNF-α、hsCRP水平呈负相关(P<0.05)。血浆外泌体miR-214-3p、miR-184单独及联合诊断IBD的AUC分别为0.831、0.819和0.912(P<0.01)。结论 IBD患者血浆外泌体miR-214-3p表达升高和miR-184表达降低均与肠道菌群分布特征及炎症因子有关,可作为辅助诊断IBD的生物标志物。 Objective To explore the distribution characteristics of intestinal microflora in the patients with inflammatory bowel disease(IBD)and their relationship with miR-214-3p and miR-184 in plasma exosomes.Methods The intestinal microflora of 98 IBD cases(group A)and 100 health examinees(group C)in the same period were detected,and the expresssions of miR-214-3p and miR-184 in plasma exosomes were detected by real-time fluorescent quantitative PCR.The number of intestinal microflora,positive detection rate of intestinal microflora and the expresssions of miR-214-3p and miR-184 in plasma exosomes were compared between the two groups.Pearson correlation method was used to analyze the relationship of the expresssions of miR-214-3p and miR-184 in plasma exosomes with the number of intestinal microflora in group A.The diagnostic value of miR-214-3p and miR-184 in plasma exosomes for IBD was evaluated by ROC curve.Results The numbers of Bifidobacterium,Bacteroides,Enterococcus,Yeast and Enterobacteriaceae were higher,while the number of Eubacterium was lower in group A than those in group C(P<0.05).The positive rates of Bifidobacterium and Yeast flora were higher,while the positive rate of Eubacterium flora was lower in group A than those in group C(P<0.05).The expresssion of miR-214-3p in plasma and serum levels of IL-6,TNF-α,hsCRP in group A were higher,while the expresssions of miR-184 in plasma was lower in group A than those in group C(P<0.05).The expresssion of miR-214-3p in plasma exosomes was positively correlated with the numbers of Bifidobacterium,Bacteroides,Enterococcus,Yeast,Enterobacteriaceae and serum levels of IL-6,TNF-α,hsCRP,and negatively correlated with the number of Eubacterium(P<0.05).The expresssion of miR-184 in plasma exosomes was positively correlated with the number of Eubacterium,and negatively correlated with the numbers of Bifidobacterium,Bacteroides,Enterococcus,Yeast,Enterobacteriaceae and serum levels of IL-6,TNF-α,hsCRP(P<0.05).The AUC of miR-214-3p and miR-184 alone and in combination in plasma exosomes for diagnosing IBD were 0.831,0.819 and 0.912,respectively(P<0.01).Conclusion The increased expresssion of miR-214-3p and decreased expresssion of miR-184 in plasma exosomes of IBD patients are related to distribution characteristics of intestinal microflora and inflammatory factors,which can be used as the biomarkers in diagnosing IBD.
