神经蛋白及调控因子在颅脑损伤中表达及意义

Expression and significance of neuroprotein and regulatory factor in craniocerebral injury

摘要 目的分析颅脑损伤(TBI)患者神经珠蛋白(NGB)、金属蛋白酶组织抑制因子-2(TIMP-2)、S100β、微小核糖核酸-124(miR-124)表达的临床意义。方法收集2021年3月至2023年3月新乡市中心医院收治的204例TBI患者临床资料,根据患者病情严重程度将其分为A组[格拉斯昏迷评分(GCS)13~15分,48例],B组(GCS评分9~12分,97例),C组(GCS评分8分及以下,59例);根据患者预后情况将其分为Ⅰ组[格拉斯预后评分(GOS)4~5分,163例]和Ⅱ组(GOS评分1~3分,41例)。采用酶联免疫吸附试验检测患者血清NGB、TIMP-2、S100β水平,采用标准曲线法测定miR-124表达量。计数资料采用χ^(2)检验、χ^(2)趋势检验比较,计量资料组间比较采用独立样本t检验、单因素方差分析,指标的预测价值采用受试者工作特征曲线(ROC)分析。结果A组血清NGB、TIMP-2、S100β、外周血miR-124水平低于B组低于C组[(68.37±11.27)ng/ml比(134.53±17.38)ng/ml比(200.68±28.59)ng/ml、(37.49±7.35)ng/ml比(45.26±8.60)ng/ml比(59.63±9.26)ng/ml、(0.89±0.23)μg/L比(1.15±0.21)μg/L比(1.42±0.30)μg/L、2.69±0.48比3.32±0.53比4.42±0.63,F=568.140、96.121、63.024、139.726,P<0.05]。Ⅱ组血清NGB、TIMP-2、S100β、外周血miR-124水平高于Ⅰ组[(152.59±49.18)ng/ml比(134.45±52.15)ng/ml、(51.35±9.75)ng/ml比(46.64±12.19)ng/ml、(1.25±0.28)μg/L比(1.14±0.31)μg/L、3.76±0.89比3.43±0.83,t=2.0127、2.295、1.973、2.270,P<0.05]。血清NGB、TIMP-2、S100β、外周血miR-124联合预测TBI患者预后的曲线下面积(AUC)值高于四者单独检测(0.895比0.786、0.775、0.761、0.773,Z四者联合-NGB=2.763、Z四者联合-TIMP-2=2.333、Z四者联合-S100β=2.598、Z四者联合-miR-124=2.592,P<0.05)。结论血清NGB、TIMP-2、S100β、外周血miR-124水平与TBI患者病情严重程度呈正相关,与预后情况呈负相关,且对患者预后具有较好的预测价值。 Objective To analyzy the clinical significance of the expression of neuroglobin(NGB),tissue inhibitor of metalloproteinase-2(TIMP-2),S100βand microRNA-124(miR-124)in patients with traumatic brain injury(TBI).Methods The clinical data of 204 TBI patients admitted to Xinxiang Central Hospital from March 2021 to March 2023 were collected.According to the severity of the patient’s condition,they were divided into group A[Glasgow coma score(GCS)13-15 points,48 cases],group B(GCS score 9-12 points,97 cases),group C(GCS score 8 points and below,59 cases).According to the prognosis of patients,they were divided into groupⅠ[Glasgow outcome score(GOS)4-5 points,163 cases]and groupⅡ(GOS score 1-3 points,41 cases).The levels of serum NGB,TIMP-2 and S100βwere detected by enzyme-linked immunosorbent assay,and the expression of miR-124 was determined by standard curve method.The enumeration data were compared byχ^(2) test andχ^(2) trend test.The measurement data were compared by independent sample t test and single factor analysis of variance.The predictive value of the index was analyzed by receiver operating characteristic(ROC)curve.Results The levels of serum NGB,TIMP-2,S100βand peripheral blood miR-124 in the group A were lower than those in the group B and lower than those in the group C[(68.37±11.27)ng/ml vs.(134.53±17.38)ng/ml vs.(200.68±28.59)ng/ml,(37.49±7.35)ng/ml vs.(45.26±8.60)ng/ml vs.(59.63±9.26)ng/ml,(0.89±0.23)μg/L vs.(1.15±0.21)μg/L vs.(1.42±0.30)μg/L,2.69±0.48 vs.3.32±0.53 vs.4.42±0.63,F=568.140,96.121,63.024,139.726,P<0.05].The levels of serum NGB,TIMP-2,S100βand peripheral blood miR-124 in the groupⅡwere higher than those in the groupⅠ[(152.59±49.18)ng/ml vs.(134.45±52.15)ng/ml,(51.35±9.75)ng/ml vs.(46.64±12.19)ng/ml,(1.25±0.28)μg/L vs.(1.14±0.31)μg/L,3.76±0.89 vs.3.43±0.83,t=2.0127,2.295,1.973,2.270,P<0.05].The area under the curve(AUC)value of combined detection of serum NGB,TIMP-2,S100βand peripheral blood miR-124 in predicting the prognosis of patients with TBI was higher than that of the indicators used alone(0.895 vs.0.786,0.775,0.761,0.773,Z four combined-NGB=2.763,Z four combined-TIMP-2=2.333,Z four combined-S100β=2.598,Z four combined-miR-124=2.592,P<0.05).Conclusion The levels of serum NGB,TIMP-2,S100βand peripheral blood miR-124 were positively correlated with the severity of TBI patients,negatively correlated with the prognosis,and had a good predictive value for the prognosis of patients.
